*National Guidelines on Malaria Diagnosis and Treatment*

1. All suspected case i.e patient presenting with fever must have parasitology test done (mRDT or microscopy)

2. Only confirm cases of malaria should be treated with ACTs to avoid resistance.

3. Assessment is either i) uncomplicated malaria or ii) severe malaria.

4. *Uncomplicated malaria* is fever, joint pain, headache, etc. PLUSE positive parasitology test.

5. *Severe malaria* is all of the above PLUS any of the following; loss of consciousness, multiple convulsion, severe anaemia, severe dehydration, hyperthermia, hyperparasitamia, haemoglobinuria, hypoglycaemia, prostration, etc.

6. National Guidelines recommends AL (first line) or AA for Uncomplicated malaria.

7. AL is prescribed as follows; stat, second dose 8hrs after first dose, third dose 24hrs after first dose, then 12hrly up to 3/7. AA is given once daily for 3 consecutive days.

8. Severe malaria is treated with IV artesunate 3mg/kg for patient < or = 20kg or 2.4mg/kg for patient > or =20kg up to 60kg (max. of 144mg per dose) given at 0hr, 12hr and 24hr, then daily until patient can tolerate orally. If patient can tolerate orally, continue ACTs as above.

9. Artesunate should be reconstituted using 1ml of HCO3 first then 5mls of water for injection giving a total of 6mls for IV route (1ml = 10mg) or 2mls water for injection giving a total of 3mls for IM route (1ml = 20mg)

10. For severe malaria do microscopy 8hly to ensure the parasite density is reducing

11. For uncomplicated malaria ensure patient comes back after in 3days for repeat microscopy to ensure his/her blood is cleared of MP

YODI AND BUM ENLARGEMENT

Survey on what attracts the male s*x to the opposite s*x revealed some interesting facts. In a physical poll conducted amongst the occupants of a males' college hostel, 70% of the occupants affirmed to prefer females with big butt. As a result of the questionable males' obsession for ladies with large butt, it’s not surprising that certain ladies have chosen to go all the length to have the “Perfect b***y”. The search for the perfect b***y has gripped women regardless of culture, economic/educational status. Some go for diet modifications, hip and butt enlargement exercises. Those who can’t do these abuse certain medication with the hope of achieving a bigger butt completing neglecting the consequences of their action.

One of such drug of abuse is DEXAMETHASONE marketed surprisingly under brand name YODI. OTHER TRADE NAMES INCLUDE DEXALAB, DECARDON, DEXASONE, p DEXAMED.

Remember 'All drugs are poisons but not all poisons are drugs'

DEXAMETHASONE is a corticosteroid used for reducing inflammation in many conditions such as rheumatoid arthritis, acute arthritis. It is also administered in severe allergic conditions that fail to respond to other treatments. The drug is a prescription only medication however it’s available over the counter at many patent medicines stores.

Side effects of DEXAMETHASONE include weakened immune system, irregular me**es, fluid retention, weight gain (especially in your face, neck, waist and buttocks), high blood pressure, headache, muscle weakness, eye problems and peptic ulcer. Apparently the side effect being weight gain (water retention in areas including the buttock area) is the major catalyst for its abuse while other side effects are disregarded by the consumers.

However those who have fallen into the trap of abuse of the drug, it can be stopped but it should be done with caution. The process of halting should be gradual, reduce the intake over weeks and months and then stop.

Dexamethasone was not formulated for butt enlargement!!!!!

Today is international day to end obstetrics fistula.. May 23.
Obstetric fistula, a condition that affects the females world wide.

About 2 million women are affected globally .

40% in Nigeria.. -


Obstetric fistula, an abnormal communication between female ge***al tract( vaginal or womb) and urinary tract or f***l tract due to obstetric causes e. g obstructed labour .it is a preventable and treatable condition.

Some of the factors that predispose to fistula formation include;

1. Early marriage, pregnancy, child birth
2. Illiteracy & ignorance
3.Poverty
4. Poor health seeking behavior s
5.delay in using health facilities

Common features are;

Leakage of urine or f***s from the female's ge***al area(private part), body smell, infections, ge***al cancers, depression, low self esteems, infertility,.etc

We can end fistula by preventing the above risk factors from occurring,including promoting female child education, stopping harmful traditional practice like cutting of female's erectile organ (cl****is), supervised labour etc..
