Inside the Editor’s Mind: What It Takes to Publish 📝

In the latest episode of Inside Surgery, host Dr. Alice Tsai goes behind the scenes of our journal, Surgical Endoscopy. She is joined by two esteemed Section Editors: Professor Young-Woo Kim (National Cancer Center Korea) and Professor Dr. Nicolo de Manzini (University of Trieste).

Together, they pull back the curtain on the editorial process, discussing their roles as the crucial "first filter" for manuscript submissions and what it truly takes to get your research published.

Key highlights include:

- The Gatekeepers of Quality: How Section Editors protect reviewers' time by ensuring only papers with clear value, relevance, and high quality move forward to peer review.

- Scientific Rigour & Innovation: Professor Kim’s insights on avoiding selection bias, the importance of prospective studies, and the future of clinically useful AI in surgery.

- Real-World Practicality: Professor de Manzini’s focus on the value of everyday surgical improvements, managing complications, safety, and navigating difficult cases.

- The Blueprint for Success: What it really means to publish in Surgical Endoscopy and how to align your research with what editors are looking for.

How many cases does an institution need to master Robotic Pancreatoduodenectomy? 🤖🏥

We know individual surgeon learning curves, but what about the whole hospital? A new study reviewed 100 consecutive RPDs at a high-volume center to find the exact institutional learning curve using RA-CUSUM analysis.

Key findings:
🔹 The Threshold: The institutional learning curve was achieved at 44 cases, and stabilized at 76 cases.
🔹 Improving Metrics: After 76 cases, operative time and major complications dropped significantly!
🔹 RPD vs. OPD: RPD showed longer OR times but resulted in shorter hospital stays and comparable safety and oncologic outcomes to open surgery.

The bottom line:
Adopting robotic pancreatoduodenectomy is safe and feasible for high-volume centers. Once the 44-case learning curve is passed, patients benefit from shorter hospital stays with outcomes comparable to open surgery. 🚀

Dealing with recurrent weight gain after a gastric bypass is a complex challenge. A new MBSAQIP database study analyzed 616 patients to compare two major revisional options: BPD/DS and SADI-S.

Key findings:
🔹 Safety First: No significant differences in 30-day complication rates, operative time, or length of hospital stay.
🔹 Patient Mix: BPD/DS patients generally presented with higher pre-op BMIs and more comorbidities like hypertension.
🔹 Early Results: Both procedures showed highly comparable short-term weight loss metrics at the 30-day mark!

The bottom line:
Both surgeries are safe, reliable options with comparable short-term outcomes for tackling recurrent weight gain after RYGB. 🏥📉
