ERN eUROGEN has published new guidance on transition care, helping ensure young people with rare uro-recto-genital conditions are properly supported as they move from paediatric to adult healthcare.

The guidance was developed by the ERN eUROGEN Transition & Lifelong Care Working Group, bringing together clinicians and patient representatives, and sets out seven recommendations for how transition should be organised, including a named coordinator and an individual transition plan for every patient.

It's intended for clinicians and multidisciplinary teams, healthcare providers reviewing their transition pathways, patient advocacy groups, and national health systems and policymakers.
