Understanding Your Rh Negative Pregnancy

A Guide for Care from Your Fetal Medicine Team

At SUBHA ULTRASOUND AND FETAL MEDICINE CLINIC, we are committed to providing you with the highest standard of care. If you have been told you have an Rh negative blood type, this brochure will help you understand what that means for your pregnancy and how we can work together, using the most advanced guidelines, to ensure the best possible outcome for you and your baby.

What Does Rh Negative Mean?
Your blood type includes a protein called the Rh factor. Most people are Rh positive, meaning they have this protein. If you do not have it, you are Rh negative. This is a common variation and does not affect your general health.

Why Does This Matter During Pregnancy?
The father of the baby may be Rh positive. If your baby inherits the Rh positive factor, it is called Rh incompatibility.

During pregnancy or birth, a small amount of the baby's Rh-positive blood could mix with your Rh-negative blood. Your body might see the baby's Rh protein as a "foreign" substance and produce antibodies to attack it. This is called sensitization.

In this pregnancy, sensitization usually does not harm the baby. However, the antibodies your body creates can stay with you. In a future pregnancy with another Rh-positive baby, these antibodies could cross the placenta and attack the baby's red blood cells, leading to anemia (low red blood cell count) or more serious conditions.

The good news is that this is almost entirely preventable.

How We Prevent Sensitization
We follow strict international guidelines (including WHO, ACOG, and FMF) to protect your pregnancy. The key is a medication called Rho(D) Immune Globulin, often referred to as "Anti-D."

This acts as a "bodyguard," clearing any of the baby's Rh-positive cells from your bloodstream before your immune system can react to them.

You will receive Rho(D) Immune Globulin at specific times:

1. Routine Antenatal Prophylaxis
Around the 28th week of pregnancy, we will give you an injection of Rho(D) immune globulin. This covers you for any small, undetected bleeds that can occur in the third trimester.

2. After Potential Sensitizing Events
You may need an additional dose of Rho(D) immune globulin any time there is a risk of your blood mixing with the baby's. This includes:
- Invasive procedures: such as amniocentesis or chorionic villus sampling (CVS).
- Bleeding: any vaginal bleeding during pregnancy.
- Abdominal trauma: such as a fall or car accident.
- External Cephalic Version (ECV): if your doctor needs to manually turn a breech baby.
- Pregnancy loss: miscarriage or termination at any stage.

3. After Birth
Immediately after your baby is born, we will test their blood type. If your baby is confirmed to be Rh positive, you will receive another dose of Rho(D) immune globulin, ideally within 72 hours of delivery .

Advanced Fetal Monitoring: MCA PSV Doppler
For patients who have already developed antibodies (are sensitized), or in certain high-risk situations, we utilize a specialized, non-invasive ultrasound technique called Middle Cerebral Artery Peak Systolic Velocity (MCA PSV) Doppler.

This advanced tool measures the speed of blood flow in the fetal brain. As outlined in SMFM (Society for Maternal-Fetal Medicine) guidelines, if a baby becomes anemic, the blood flows faster to compensate. By measuring this speed, we can accurately detect anemia without the need for invasive procedures like amniocentesis or cordocentesis.

What MCA PSV Monitoring Means for You:
- It is the Gold Standard: MCA PSV is now the primary technique recommended by experts to detect fetal anemia. It has replaced older, more invasive methods.
- Early and Safe Detection: If monitoring shows that the MCA PSV rises above 1.5 times the median (1.5 MoM) for your gestational age, it is a strong indicator of moderate or severe anemia.
- Guiding Treatment: This information allows our fetal medicine specialists to act immediately. A result above this threshold typically indicates the need for a fetal blood sampling and intrauterine transfusion (IUT) to correct the anemia.
- Monitoring After Transfusion: For babies who require one transfusion, MCA PSV is also used to determine the optimal timing for any subsequent transfusions. Studies show that using MCA PSV has led to better outcomes, including a reduction in fetal hydrops and improved survival rates.

Your Personalized Care Plan
As a Fetal Medicine Clinic, we create an individualized plan for you, which may include:

- Initial & Ongoing Antibody Screening: Your blood will be checked at your first visit and regularly throughout pregnancy to detect if any antibodies have developed.
- Fetal DNA Testing: In some cases, we can use a simple maternal blood test to determine the baby's blood type (Rh status) early in pregnancy. If the baby is found to be Rh negative like you, no further treatment is needed.
- MCA PSV Surveillance: If antibodies are present, you will undergo regular, scheduled MCA PSV Doppler scans (often weekly) to continuously monitor the baby's wellbeing .
- Kleihauer-Betke Test: After a significant bleed or trauma, this test measures the volume of fetal blood in your system to ensure we give you the precise dose of Rho(D) immune globulin needed for full protection .

What is a Fetal Echocardiogram?
A fetal echocardiogram ("fetal echo") is a specialized, detailed ultrasound scan that focuses solely on your baby’s developing heart. It is not a routine scan, but a targeted examination performed by our expert fetal medicine specialists and cardiologists.

When is it Recommended?
Your doctor may recommend this detailed scan if there is a reason to look more closely at the baby's heart. Common indications include:
For You (Maternal Factors):
Diabetes (Type 1, Type 2, or poorly controlled)
Autoimmune diseases (e.g., Lupus)
Certain medications or infections during pregnancy
Pregnancy via IVF/ICSI
For Your Baby (Fetal Factors):
Suspected heart issue on your routine ultrasound
Increased nuchal translucency (NT) in first trimester

Twins, especially identical (monochorionic) twins

What to Expect During Your Scan
Timing: The scan is best performed between 18 and 24 weeks of pregnancy, when the heart is developed enough for clear imaging.
The Procedure:
1. Duration: The scan typically takes 30-45 minutes.
2. Process: Much like your anatomy scan, you will lie comfortably. Our specialist will apply gel and use an ultrasound transducer on your abdomen. We use advanced Doppler and 3D/4D technology to obtain the best images.
3. Comfort: The procedure is safe and painless for both you and your baby.
4. Results: In most cases, our specialist will discuss the findings with you immediately after the scan. If a detailed pediatric cardiology opinion is needed, we will coordinate that for you.
