ENDOMETRIOSIS AND INFERTILITY:

Endometriosis is a disease that affects women of reproductive age and that may be associated with both pelvic pain and infertility. Scientific advances have improved our understanding of this benign (non-cancerous) but sometimes debilitating condition. And modern medicine now offers women with endometriosis many treatment options for relief of both pain and infertility.

But much remains unknown. Women who have been diagnosed with endometriosis may wonder what this means for their future fertility. Similarly, women experiencing infertility may wonder whether they have endometriosis and how that may affect their prognosis.

what is infertility - infertility is defined as the inability to conceive, or become pregnant within 1 full year of actively trying or after two or more miscarriages. Over 90 million couples worldwide are trying to conceive at any given time. Out of these couples, 1 in 6 (17%!) experience infertility troubles. When no fertility problems are present, the average couple between ages 29 and 33 has about a 20 to 25 percent chance of becoming pregnant during any given menstrual cycle. Unfortunately, this probability decreases dramatically by one third to one half as women approach their mid 30's and early 40's.

Common causes of male infertility
The “male factor” contributes to infertility around half the time, and about one third of the time, it's the main cause of infertility. Most often, the problem lies in the process of either making or moving thes***m.
Any of the following can cause a man to have a low s***m count or abnormal s***m:
• Varicocele — an abnormal collection of bulging veins above the testicle; they’re the most common cause of correctable male infertility, accounting for 38% of cases
• Undescended testicle
• Infections in the testicle (orchitis), the prostate (prostatitis), or elsewhere in the body that causes a fever
• Chemotherapy for cancer
• Medicines such as anabolic steroids or anti-seizure medicines
• Genetic abnormalities
• Hormone problems
In some cases, these problems can be reversed, but other times they can’t. An evaluation by a physician is the only way to sort it out.

ertility treatments - Take time to plan
Before you and your partner start treatment, talk about how far you want to go with treatment. For example, you may want to try medicine but don't want to have surgery. You may change your mind during your treatment, but it's good to start with an idea of what you want your limits to be.
Treatment for fertility can also cost a lot. And insurance often doesn't cover these expenses. If cost is a concern for you, ask how much the medicines and procedures cost. Then find out if your insurance covers any costs. Talk with your partner about what you can afford.
Thinking about this ahead of time may help keep you from becoming emotionally and financially drained from trying a series of treatments you hadn't planned for.

Diagnosis and Treatment of Unexplained Infertility
Over the past decade, significant advances have occurred in the diagnosis and treatment of reproductive disorders. In this review, we discuss the routine testing performed to diagnose unexplained infertility. We also discuss additional testing, such as assessment of ovarian reserve, and the potential role of laparoscopy in the complete workup of unexplained infertility. Finally, we outline the available therapeutic options and discuss the efficacy and the cost-effectiveness of the existing treatment modalities. The optimal treatment strategy needs to be based on individual patient characteristics such as age, treatment efficacy, side-effect profile, and cost considerations.

RECURRENT PREGNANCY LOSS
Recurrent Pregnancy Loss (RPL) is a condition, distinct from infertility, defined by two or more failed pregnancies. When the cause is unknown, each pregnancy loss merits careful review to determine whether specific evaluation may be appropriate. After three or more losses, a thorough evaluation is warranted.

Traditionally, the diagnosis of Recurrent Loss is not made until a woman has lost at least three pregnancies. Overall about 20% of pregnancies end in miscarriage due to a non-recurrent causes while 4% of women will experience two losses. The odds of three losses would be 0.16%. This means that if you have lost three pregnancies it is quite unlikely that this is due to three abnormal pregnancies. More likely one specific abnormality or underlying condition resulted in all three losses. And, if undetected or untreated, you may likely be at increased risk for a loss in a subsequent pregnancy.

Is infertility just a woman's problem?

No, infertility is not always a woman's problem. Both women and men can have problems that cause infertility. About one-third of infertility cases are caused by women's problems. Another one third of fertility problems are due to the man. The other cases are caused by a mixture of male and female problems or by unknown problems.

Fertility Problems - Treatment Overview
Some fertility problems are more easily treated than others. In general, as a woman ages, especially after age 35, her chances of getting pregnant go down. But her risk of miscarriage goes up.
If you are 35 or older, your doctor may recommend that you skip some of the steps younger couples usually take. That's because your chances of having a baby decrease with each passing year.
It's important to understand that even if you are able to get pregnant, no treatment can guarantee a healthy baby. On the other hand, scientists in this field have made many advances that have helped millions of couples have babies.
Take time to plan
Before you and your partner start treatment, talk about how far you want to go with treatment. For example, you may want to try medicine but don't want to have surgery. You may change your mind during your treatment, but it's good to start with an idea of what you want your limits to be.
