Before you complete your enrollment, make sure you are informed about the medical requirements and clearance procedures of the PUP Alfonso Campus Medical Services Department.

Your health documents are an important part of your admission requirements. Ensure that all forms are properly accomplished and all supporting documents are complete before submission.

📢 ATTENTION, INCOMING FRESHMEN ISKOLARS! 🎓💛❤️

Welcome to PUP Alfonso Campus!

As part of the enrollment and student health clearance requirements, all incoming freshmen are required to accomplish and submit the following documents:

✅ Chest X-Ray Official Result
✅ Medical Certificate (issued by a licensed physician indicating that you are physically fit to study or enroll in your chosen program)
✅ Health Information Form for Students (attach a recent 2×2 colored ID picture and accomplish Part I ONLY)
✅ Dental Health Record Form (the student should fill out the required information, while the last part must be completed and signed by a licensed dentist)
✅ Student Medical Waiver Form
✅ One (1) Long Brown Envelope (Blank)

📌 IMPORTANT REMINDERS:
• Print all forms on A4-size paper.
• Fill out all forms completely and accurately.
• Use clear and legible handwriting.
• Ensure all required signatures are affixed.
• Keep both photocopy and digital PDF copies of all submitted documents for your records.
