❓Who: Fourth Year Student Teachers
❓When: October 13 and 14, 2026 9:00 AM to 3:00PM
❓Where: Room 107 URS Angono Campus

1. Attached is the letter of Medical Dental Exam. Kindly read😁.

2. Attached also is a copy of the Laboratory Request form (Male and Female). The following laboratory procedure must be submitted:
CBC
Urinalysis
Drug Testing
Chest Xray PA view
Pregnancy Test (for female only - the test will be done at the URS Angono Clinic, please bring your own Pregnancy Test Kit)‼️

3. The schedule for the Initial Assessment of your Medical requirements is on October 6-8, 2026 9AM to 3PM. Please bring Original and PHOTOCOPY of your results. ‼️

4. Our University Physician and Dentist will be present on October 13-14, 2026 for your medical examination and evaluation.

5. Should you have inquiries or clarifications kindly message your Campus Nurse‼️ (Monday - Thursday 8AM to 4PM)

What: MEDICAL AND DENTAL EXAMINATION FOR OJT

When: SEPTEMBER 8, 2026

TIME/VENUE: ROOM 107

Mangyari po lamang na pumunta sa URS ANGONO Clinic para makakuha ng lab request at form

Kapag napagawa nyo na po yung lab request.
Magphotocopy po ng tig isa. I fill up po ang Medical and Dental Certificate form.

Ibalik po agad sa clinic ang results at photocopy, para sa inyong initial assessment. Ito po ay dapat isubmit on September 2 and 3, 2026. .

Ang Pregnancy test (for females only) ay sa ating campus clinic po gagawin. Bumili lang ng pregnancy test kit.

Kung may mga katanungan po ay approach nyo lang po si nurse o message po sa page. Maraming salamat po ☺️
