Madaling sundan:

STEP-BY-STEP NA MECHANISM

1️⃣ Mabilis na fluid removal (Ultrafiltration)
Sa dialysis, tinatanggal ang sobrang tubig sa dugo
Kapag mabilis ang UFR, bumababa ang blood volume
👉 Resulta: hypovolemia

2️⃣ Bumaba ang blood pressure
Dahil kulang ang circulating volume
Hindi agad nakaka-compensate ang katawan
👉 Resulta: intradialytic hypotension

3️⃣ Kulang ang daloy ng dugo sa muscles
Mas inuuna ng katawan ang vital organs (brain, heart)
Naiiwan ang skeletal muscles
👉 Resulta: muscle ischemia (kulang sa oxygen)

4️⃣ Electrolyte at osmolar shifts
Nagkakaroon ng pagbabago sa:
Sodium
Calcium
Osmolality

👉 Resulta: increased neuromuscular excitability
5️⃣ Biglang contraction ng muscle
Dahil sa ischemia + electrolyte imbalance
Nagiging hyperexcitable ang muscle fibers
👉 Resulta:
❗ Sustained involuntary contraction = CRAMPS

BLOOD LEAK!!
(Mga kailangan mong tandaan sa gantong situation)

🚨 BLOOD LEAK ALARM: NURSING ACTIONS
🔴 1. STOP the blood pump immediately
Prevents further blood loss
Limits contamination
🔴 2. DO NOT return the blood
Blood may be contaminated with dialysate
Returning it can cause hemolysis or infection
🔴 3. Clamp the bloodlines
Clamp both arterial and venous lines
Prevent air entry and blood loss
🔴 4. Inspect the system
Check dialyzer for:
Visible cracks or rupture
Pink/red discoloration in dialysate outflow
Confirm if it’s a true vs false alarm
🔴 5. Terminate dialysis
Safely disconnect patient using aseptic technique
Do NOT reinfuse extracorporeal blood
🔴 6. Monitor the patient closely
Watch for:
Hypotension
Chest/back pain
Shortness of breath
Signs of hemolysis
🔴 7. Notify physician immediately
Further orders may include labs (e.g., hemoglobin, hemolysis workup)
🔴 8. Document & report
Time of alarm
Actions taken
Patient condition
Machine/dialyzer details
