YAKAP FACILITY
Good day! The Bilar Primary Care Facility is now transitioning to the PhilHealth Konsulta (Yakap) system. Bilar Primary Care Facility is officially a Yakap Facility and open for registration of PhilHealth members and their dependents.
This new system aims to provide better, organized, and continuous health services for our patients.

๐ What to Bring During Consultation & Other Services;
To ensure a smooth process, please bring the following:

PhilHealth ID or PhilHealth Number
โ
MDR (Member Data Record), if available
โ
National ID (Preferred)
โ
Any Valid ID if PhilSys / National ID is not available

๐ CONSULTATION PROCESS
1๏ธโฃ Arrival at the Facility
โข Upon arrival, get your priority number.

2๏ธโฃ ID Verification
โข Staff will ask for:
โข PhilHealth ID or MDR
โข National ID or any valid ID

3๏ธโฃ Facility Registration
โข Staff will check if you are already registered with a Konsulta/Yakap Facility.
If NOT registered:
You will be registered to Bilar Primary Care Facility.

Once registered, you will be assigned (locked-in) to our facility for one (1) year.

5๏ธโฃ PhilHealth Check Utility (Liveness Verification)
To save time, you may do this ON THE DAY OF YOUR CONSULTATION.
Follow these steps:

2๏ธโฃ Fill out the required information
3๏ธโฃ Start the Liveness Check
4๏ธโฃ If successful:
๐ธ Screenshot the result and show it to the health worker.
5๏ธโฃ If ERROR occurs:

The health worker will then:
โข Take a photo of you
โข With your valid ID
โ Purpose:
This verifies the liveness of the patient and confirms that the person seeking care is the actual PhilHealth member/dependent.

The Health Care Worker will assist you in signing:

By signing this:
โ You agree to be served by our facility for one year
โ You may avail available Konsulta services for FREE
