Good day, Students!

Please be informed that the SASO–School Clinic will begin the medical assessment of all 1st Year and 2nd Year college students who will be participating in the upcoming PE Demonstration for the Founder's Day Celebration, starting July 27, 2026.

To ensure an orderly and efficient assessment process, please follow the assigned schedule below:

Students are requested to report to the School Clinic only during their assigned schedule to avoid overcrowding and delays.
Please complete this medical assessment and secure your Clinic Confirmation Slip before participating in any PE practice sessions.

PhilHealth YAKAP Program Enrollment and PhilHealth Benefits

Good day, everyone!

During our Employee Assembly held on July 17, 2026, the SASO- School Clinic shared an update regarding the PhilHealth YAKAP Program. We encourage all employees to enroll and take full advantage of the healthcare benefits available through PhilHealth. As part of the School Clinic's initiative to promote employee health and wellness, we are encouraging everyone to complete their enrollment in the PhilHealth YAKAP Program. This initiative also aims to help reduce the school's healthcare-related operating costs, particularly those associated with the Annual Medical Examination (AME) of employees, by maximizing the healthcare services and benefits available through PhilHealth.
Steps to Enroll in the PhilHealth YAKAP Program
1. Visit the PhilHealth website: www.philhealth.gov.ph.
2. Click Online Services.
3. Select Member Portal.
4. Complete the Member Portal Account Creation Page by providing the required information.
5. Activate your account by clicking the activation link sent to your registered email address.
6. Once your account has been activated, log in to the PhilHealth Member Portal.
7. After logging in, select your PhilHealth YAKAP Partner Clinic/Hospital.
8. Choose Cebu Provincial Hospital – Bogo City as your designated PhilHealth YAKAP Partner Hospital.

Once You Have Selected Your Partner Hospital After selecting Cebu Provincial Hospital – Bogo City as your PhilHealth YAKAP partner hospital, please remember to:
• Keep your personal and PhilHealth records updated.
• Bring a valid government-issued ID and your PhilHealth Number during consultations or hospital visits.
• Inform the healthcare provider that you are a PhilHealth member so your benefits can be processed accordingly.
• Follow your scheduled appointments and the instructions provided by your healthcare team.
• Ask the healthcare provider about the PhilHealth-covered services available to maximize your healthcare benefits.

Note: If you do not know your PhilHealth Identification Number (PIN), you may request assistance from the School Clinic or Ms. Melodina Fernandez from the Accounting Department before creating your Member Portal account.

Laboratory Tests covered by Philhealth Yakap:
1. CBC with platelet count
2. Urinalysis
3. F***lysis ( Stool Exam)
4. Sputum Microscopy
5. F***l Occult Blood Test
6. Pap Smear
7. Lipid Profile ( Triglycerides, Total Cholesterol, HDL, LDL)
8. Fasting Blood Sugar (FBS)
9. Oral Glucose Tolerance Test (OGTT)
10. ECG
11. Chest X-ray
12. Creatinine
13. Glycosylated Hemoglobin ( HBA1c)

Diagnostic Tests:
1. Breast Cancer ( Breast Ultrasound, Mammogram)
2. Liver Cancer ( Alpha-fetoprotein, Liver Ultrasound)
3. Lung Cancer ( Low dose chest, CT Scan)
4. Colorectal Cancer ( Colonoscopy)

Note: An annual allocation of up to ₱20,000 for essential outpatient medicines, subject to the PhilHealth YAKAP prescribing guidelines and the evaluation and prescription of the attending physician.

Important Reminders:
Employees who have already selected a different PhilHealth YAKAP partner healthcare provider may continue to avail of their healthcare services from their existing registered partner provider. There is no need to change your designated partner hospital unless you wish to do so and are allowed under PhilHealth's policies.

For employees who have not yet selected a PhilHealth YAKAP partner provider, please choose Cebu Provincial Hospital – Bogo City during your enrollment in the PhilHealth Member Portal. This will support the School Clinic's initiative to maximize PhilHealth benefits while helping reduce the school's healthcare-related operating costs, particularly during the Annual Medical Examination (AME) of employees.

Your participation in this initiative is highly encouraged, as it not only allows you to maximize your PhilHealth benefits but also supports the School Clinic's efforts to provide quality healthcare services while helping reduce the school's healthcare-related operational costs, particularly during the Annual Medical Examination (AME) of employees.

📢 SCHOOL CLINIC HEALTH ADVISORY | STOMACH FLU (Gastroenteritis) 💙

Dear Students, Parents, and Guardians,

Your health and well-being are our priority. As we continue to promote a safe and healthy school environment, we encourage everyone to be mindful of stomach flu (gastroenteritis) and take simple steps to prevent its spread.

Common signs and symptoms include:
🤢 Nausea and vomiting
💩 Diarrhea
🤕 Stomach cramps or abdominal pain
🌡️ Fever (in some cases)

Help protect yourself and others by:
🧼 Washing your hands regularly with soap and water, especially before eating and after using the restroom.
💧 Drinking safe, clean water and staying hydrated.
🍎 Eating clean, freshly prepared, and properly cooked food.
🚫 Avoiding sharing water bottles, utensils, and other personal items.
🧹 Keeping your surroundings clean and practicing good personal hygiene.

If you are feeling unwell:
🏠 Stay at home and get enough rest.
🥤 Drink plenty of fluids or an Oral Rehydration Solution (ORS) if you have diarrhea or vomiting.
👨‍⚕️ Seek medical attention if your symptoms become severe, such as persistent vomiting or diarrhea, high fever, severe abdominal pain, or signs of dehydration.

📚 Students who are experiencing symptoms are encouraged to stay home until they have fully recovered to help prevent the spread of illness and protect the health of our school community.

To those students who weren't able to attend their Medical Assessment schedule, you're being re-scheduled on December 23, 24 and 26, 2025.
Reminders.
✅ Please bring the following requirements:
· Your signed Waiver(Attached with photo copy of the parent/guardian valid Identification, with three(3) specimen signatures).
· Your School ID (NO ID, NO ASSESSMENT).
⏰ Time Rules:
· Morning Start: 8:30 AM
· Morning Cut-off: 10:00 AM
· Afternoon Start: 1: 30 PM
· Afternoon Cut-off: 3:00 PM (Be on time.)
📌 To avoid delays:
· Have all your documents ready before you arrive.
· Follow all instructions and maintain good behavior.
