PhilHealth YAKAP beneficiaries may claim the following medicines covered under the PhilHealth YAKAP program with a valid electronic prescription and UPSC code issued by their accredited Primary Care Provider (P*P).

ℹ️ Please note: PhilHealth YAKAP covers additional medicines that are not included in our current list. These medicines are not yet available at Evelyn Pharmacy, but we continue to expand our inventory to better serve our patients.

Paano mag-register sa PhilHealth YAKAP gamit ang eGovPh app? 💊

Step 1:
I-download ang eGovPH app sa Google Play o App Store.

Step 2:
Gumawa ng account at i-verify ang iyong mobile number at email. Kung mayroon ka ng account, mag-log in na lamang.

Step 3:
Sa Home screen, piliin ang NGAs at hanapin ang PhilHealth. I-tap ang PhilHealth Member Portal.

Step 4:
Piliin ang YAKAP tab, pagkatapos ay i-tap ang Add YAKAP Provider.

Step 5:
Hanapin at piliin ang iyong nais na YAKAP Provider.

Step 6:
I-confim ang iyong napiling provider. Tapos na ang iyong registration!

Step 7:
Maaari ka nang magpakonsulta sa napiling YAKAP Provider.

Step 8:
Pumunta sa EVELYN PHARMACY at i-presenta ang electronic prescription para ma-claim ang mga covered na gamot.

📌 How to Avail PhilHealth YAKAP Benefits

🟣 STEP 1: Register
Enroll in the PhilHealth YAKAP Program through any of the following:
• 📱 eGovPH App or PhilHealth Member Portal
• 🏢 Any PhilHealth Local Health Insurance Office (LHIO)
• 🩺 Your chosen PhilHealth YAKAP-accredited healthcare provider

🟣 STEP 2: Consult
Visit your chosen PhilHealth YAKAP-accredited Primary Care Provider (P*P) for consultation.

🟣 STEP 3: Get an Electronic Prescription

If you're eligible for covered medicines, your healthcare provider will:

✅ Issue an electronic prescription
✅ Provide your UPSC (Unique Prescription System Code)

🟣 STEP 4: Claim Your Medicines
Proceed to Evelyn Pharmacy and present:
✔️ Your UPSC
✔️ One (1) valid government-issued ID

If a representative will claim the medicines on your behalf, please present:
✔️ An authorization letter signed by the patient
✔️ A valid ID or supporting document showing the representative's relationship to the patient
