Attention Incoming 2nd and 3rd Year Students and Enrollees of Cebu Technological University – Consolacion Campus

As part of the enrollment requirements, all enrollees are required to submit the following medical and laboratory examination results to the Campus Clinic/Health Services:

- Chest X-Ray
- Stool Exam

Please take note of the following:

• Medical requirements must be secured from DOH-accredited medical facilities.
• Medical examination results must not be more than six (6) months old from the date of testing/examination.
• The clinic will only accept PHOTOCOPIES of the medical results. Kindly keep the original copies for your personal record unless otherwise requested.

Please ensure that all submitted documents are complete and clear to avoid delays in processing your enrollment requirements.

- Complete Physical Examination with Medical Certificate

Please be informed that the SIS Portal is currently DOWN.

In line with this, students who are Batch 1 qualifiers (late enrollees) are advised to proceed first to the Clinic after securing the necessary signatures from the Program Chair and/or Dean on their enrollment form.
Students may visit the MIS Office once the SIS Portal becomes available again.
