ANG HEALTH CARD na pang masa ay narito na, mula sa Getwell Health Systems, Inc. Sa murang halaga lamang ay magkakaroon ka na ng sumusunod:
1. FOR IN PATIENT BENEFITS
(Hospital Confinement)
A. Confinement in a GETWELL accredited hospitals.
You may avail the following services:
• room and board (depending on chosen program)
• operating room, anesthesia and recovery room
• professional services of specialists
• blood transfusions and intravenous fluids (except blood products, cross matching, blood typing)
• x-ray, laboratory and other, diagnostic examinations
• dressings, plaster casts, sutures and other items related to the management of the patient
• other coverable hospital services, when deemed necessary by designated attending physician.
• ICU confinement (Intensive Care Unit)
• CT Scan & ultrasound (computed tomography)
• MRI (Magnetic Resonance Imaging)
B. OUT-PATIENT HEALTH CARE SERVICES
• Emergency cases not requiring confinement
• Out-patient Services
• Medical consultations during clinic hours
• Treatment of minor injuries or illness
• Laboratory, X-ray and other basic diagnostic examinations prescribed by aJliated doctor
• Referral to specialists
• Eye, Ears, Nose and Throat Treatment
C. PREVENTIVE HEALTH CARE SERVICES
• Basic APE, after payment of 12 months membership fee, venue to be designated by GHSI
• Chest x-ray
• Urinalysis
• Fecalysis
• Physical exam
• Complete Blood Count (CBC)
• Immunization, except cost of medicine and vaccine
• Counselling on health habits, diet and family planning
• Recording and review of medical history
D. EMERGENCY CARE SERVICES
• If confined in an affiliated hospital, covered 100%
• If confined in non-affiliated hospital in areas with no medical provider, reimbursement 100%
• If confined in non-affiliated hospital in areas with medical provider, reimbursement 80%
• The company, member or family member must inform GETWELL within 24 hours from the time the patient is in the hospital so that reimbursement will be granted
E. DENTAL CARE SERVICES
• Oral Prophylaxis, once a year after payment of 12 months membership fee
• Unlimited simple tooth extraction
• Consultations and Oral examination
• 2 permanent fillings per year
• Treatment of lesions and wounds
F. FINANCIAL ASSISTANCE IN CASE OF DEATH. GETWELL will provide a financial assistance in case of death of the member
