Kung wala pa at hindi kayang magbayad sa buwanang premium, kumuha nga certificate of indigency sa barangay at dalhin ito sa Municipal Social Development Office (MSWDO) Kasama ang valid ID at bigyan ka ng Certificate of Indigency.

Punta sa health center at dalhin ang sumusunod:
1. Birth Certificate (photocopy)
2. Marriage Certificate (photocopy)
3. Certificate of Indigency ng MSWDO
4. Valid ID

How do organizations survive without funds?

Culaba HDC survived in a decade with monthly dues. Government support is not sustainable and sustainable. What is sustainable is individual health empowerment, individual health seeking behavior to prevent expense on hospitalization.
Procurement and supplies are delayed, not regular and not enough but maintenance must be regularly taken completely. HDC helped the out of pocket expense at a lower cost than buying individually.

EMPOWERMENT BEGIN ITSELF.
