Here are some diagrams that explain why colostomy prolapse may happen. When looking at the colon, some portions of it are fixed into the abdominal wall, and other portions are mobile. If we open a colostomy in a mobile portion, the stoma will have a tendency to prolapse. When we open the colostomy near a fixed portion, prolapse will not happen.
Unfortunately, almost the entire small bowel is mobile (not fixed to the abdominal wall) and that is why ileostomies have a tendency to prolapse. We try to avoid that from happening by fixing the ileum, proximal to the stoma, into the abdominal wall.
