2. Piliin ang serbisyo na nais ninyong i-avail.
3. Piliin ang petsa o schedule ng inyong appointment.
4.Ihanda at dalhin ang mga sumusunod na requirements:
a. Photocopy ng dalawang (2) valid IDs
b. Birth Certificate (para sa pedia at minor patients)
5. Maaaring isumite nang mas maaga ang inyong requirements bago ang schedule ng appointment o ipsa sa araw ng appointment
