Frequently Asked Questions

1. Can we use the medical certificate issued by the government physician who examined us?

• Yes — you may use it as long as the examiner is a government physician.

2. Do we need to visit the clinic for validation?
• Only first-year incoming students must have their documents validated in person. Validation is required before the registrar accepts enrollment requirements.

3. Can we ask for a PDF copy or scan of the BUkSU official medical certificate?

No. The BUkSU official medical certificate should only be released and controlled by those officially authorized to issue it. You may use the medical certificate issued by the government physician instead.

4. The government physician not from the City Health Office signed the official BukSU Medical Certificate.

The BukSU Medical Form is accepted by the City Health Office and it has a validity for one semester. The City Health Office is aware of this since they received an official communication from the University.
Some government doctors, however, may not sign or use the BukSU form because the medical certificates they issue are only valid on the day you are examined, not for your fitness for the entire semester.

If your medical examination is done by another government physician, the University Physician/Medical Staff will review your submitted medical records (BukSU Medical Form, laboratory results if available, and medical certificate) through OVPVASSS to determine if you are medically fit for the semester. If everything is complete and acceptable, you will be issued a Certificate of Compliance the after to be used for in and off campus activities.

You do not need to visit the University Clinic unless you are contacted for verification or you have existing medical conditions (comorbidities) or a special endorsement.

The form or medical certificate issued by the government physician is already valid for enrollment.

Can we ask the medical clinic to issue us a medical certificate?
No. The University Clinic caters officially enrolled students for issuance of medical certificate.

Process flow for Medical Certificate Requirements for Enrolment

For 1st Year Students

1. Obtain the following forms:
* BukSU Medical Certificate Form with semester validity (for students getting their medical certificate at the City Health Office)
* BukSU Medical Record Form
* You may also submit any medical certificate issued by a government physician who examined you.
* The medical certificate must include the remark: “Fit for in-campus and off-campus activities.”

2. Complete the required laboratory examinations:
* Complete Blood Count (CBC)
* Urinalysis
* Additional course-specific requirements (e.g., Nursing, Hospitality Management, etc.), if applicable.

3. Submit the following to the Medical Clinic (Main Campus) for validation and profiling:
* Copy of the Medical Certificate
* Medical Record Form

4. Submit a copy of the Medical Certificate to the Registrar.

5. Upload a scanned copy or electronically validated Medical Certificate through the prescribed enrollment platform or submission channel.

6. Submit the original hard copies to your respective department during the first week of classes:
* Medical Certificate

7. The OVPCASSS shall verify the submitted documents and prepare the corresponding Certificate of Compliance for approval.

8. The approved Certificate of Compliance shall serve as proof of compliance with the submission of notarized consent forms and medical certificates for participation in off-campus and in-campus activities.

For 2nd–4th Year Students

1. Obtain the required forms:
* BukSU Medical Certificate Form (if getting examined at the City Health Office)
* Medical Record Form (only if you do not yet have a record at the Medical Clinic)
* You may also submit a medical certificate issued by a government physician who examined you.
* The medical certificate must include the remark: “Fit for in-campus and off-campus activities.”

2. Laboratory examinations as deemed necessary by the government physician conducting your fitness assessment.

3. Upload a scanned copy or electronically signed Medical Certificate through the prescribed enrollment platform or submission channel.

4. Submit the original hard copies of your medical records to your respective department during the first week of classes:
* Medical Certificate
* Medical Record Form (placed in a long white folder if you do not yet have a record at the Medical Clinic.
(laboratory results if available)

5. The OVPCASSS shall verify the submitted documents and prepare the corresponding Certificate of Compliance for approval.

6. The approved Certificate of Compliance shall serve as proof of compliance with the submission of notarized consent forms and medical certificates for participation in off-campus and in-campus activities.
