⚠️ FOR CLIENTS WITH PAS APPOINTMENTS FROM AUGUST 18–20 ONLY who were unable to get their ARV refill.

📢 H4 OPD ADVISORY – AUGUST 19, 2026 (WEDNESDAY)

Due to expected heavy rains and possible flooding, H4 OPD will adjust its schedule tomorrow for the safety of our patients and staff.

ARV REFILL

* Start: 7:00 AM
* Cut-off: 11:00 AM
* All transactions should be completed by 2:00 PM

PAS Patients

* August 18 patients advised to return tomorrow: Please come early. Cut-off is 11:00 AM.
* August 19 patients: We will accommodate AM PAS appointments (8:00–11:00 AM).
* NO WALK-INS will be accommodated. Please book an appointment through PAS.

IMPORTANT: Bring your complete OHAT/PhilHealth requirements, already printed or photocopied:

* MDR
* PhilHealth ID
* PhilHealth contribution/receipt, if applicable
* HIV Confirmatory Result
* ARV Booklet

Incomplete requirements will not be accommodated. Printing and photocopying will not be available at H4 OPD.

Please come early, but do not risk traveling through heavy rain or flooded areas. Safety comes first.

📢 ADVISORY TO OUR CLIENTS

Due to the current weather conditions and flooding, please be advised that there will be NO H4 OPD tomorrow, August 18.

All clients with scheduled PAS appointments for ARV refill on August 18 will be rescheduled to Wednesday, August 19, or Thursday, August 20.

Please prioritize your safety and do not risk traveling through heavy rains or flooded areas. We understand the importance of your ARV refill, and we will make sure that affected scheduled clients are accommodated on the rescheduled dates.

We also kindly ask everyone not to walk in. Please make sure to book a PAS appointment so we can properly accommodate and prioritize scheduled clients.

Important reminders:

• ❌ August 18 (Tuesday): NO H4 OPD
• ✅ August 19–20 (Wednesday–Thursday): Rescheduled PAS appointments
• ❌ August 21 (Friday): NO REFILL – Holiday

H4 OPD Department Advisory

Please be informed that the H4 OPD Department will be CLOSED on May 27, 2026 (Wednesday) in observance of the holiday Eid’l Adha (Feast of Sacrifice).

Ang mga clients na naka-schedule sa PAS ngayong May 27,2026 (Wednesday) ay maaaring pumunta sa May 25, 2026 (Monday) para sa refill, kasunod pa rin ng kanilang original na schedule/oras.

Paalala lamang po:
- Walang tatanggapin na walk-in refill sa Monday.
- Pakisunod po ang inyong nakatalagang PAS schedule/oras.
- Ang mga pipiling pumunta ng Tuesday o Thursday ay ituturing na walk-in.

Maraming salamat po sa inyong pang-unawa at kooperasyon.
