Trigeminal Neuralgia: Causes, Signs & Risk Factors

Trigeminal neuralgia is a chronic facial pain disorder affecting the trigeminal nerve, which carries sensation from the face to the brain. It causes sudden, severe, electric-shock-like facial pain.

🟣 Common Causes

➟ Most commonly caused by a blood vessel pressing on the trigeminal nerve near the brainstem

➟ Multiple sclerosis can damage the nerve’s protective covering

➟ Rarely, a tumor or other structural problem may compress the nerve

➟ Sometimes no clear cause is found

🟣 Common Signs and Symptoms

➟ Sudden, severe, stabbing or electric-shock-like facial pain

➟ Pain usually affects one side of the face

➟ Attacks may involve the cheek, jaw, teeth, gums, lips, or around the eye

➟ Episodes usually last from a few seconds to about 2 minutes

➟ Attacks may occur repeatedly throughout the day

🟣 Common Triggers

➟ Light touching of the face

➟ Brushing the teeth

➟ Chewing or eating

➟ Talking or smiling

➟ Washing the face or shaving

➟ Cold air or wind touching the face

🟣 Typical Pain Pattern

➟ Pain often occurs in sudden bursts with pain-free periods between attacks

➟ Some people later develop a more continuous aching or burning pain between attacks

➟ Symptoms may disappear for weeks or months and then return

🟣 Risk Factors

➟ Increasing age, especially over 50 years

➟ Multiple sclerosis

➟ Conditions causing compression or damage of the trigeminal nerve

➟ Trigeminal neuralgia is somewhat more common in women

🟣 Diagnosis

➟ Diagnosis is mainly based on the characteristic pain pattern

➟ Neurological examination helps assess facial sensation and other nerve functions

➟ MRI may be recommended to look for multiple sclerosis, tumors, or nerve compression

➟ Dental causes of facial pain should also be ruled out when appropriate

🟣 Management

➟ Carbamazepine is commonly the first-line medicine

➟ Oxcarbazepine is another commonly used option

➟ Other nerve-pain medicines may be considered if symptoms persist

➟ Blood tests may be required to monitor some medications

➟ Severe or medication-resistant cases may benefit from procedures or surgery to relieve pressure on the nerve

➟ Repeated electric-shock-like facial pain

➟ Pain makes eating, drinking, or brushing teeth difficult

➟ Facial pain becomes more frequent or severe

Perforated Eardrum: Causes, Signs & Management

A perforated eardrum, or tympanic membrane perforation, is a hole or tear in the thin membrane separating the ear canal from the middle ear. Many small perforations heal on their own, but some need specialist treatment.

🟣 Common Causes

➟ Middle-ear infection causing pressure and rupture of the eardrum

➟ Sudden pressure changes from flying, diving, or explosions

➟ Direct injury from cotton buds, hairpins, or other objects placed in the ear

➟ Head or ear trauma

➟ Very loud noise or blast injury

🟣 Common Signs and Symptoms

➟ Sudden ear pain, which may improve after the eardrum ruptures

➟ Reduced or muffled hearing

➟ Ringing in the ear, called tinnitus

➟ Fluid, pus, or blood draining from the ear

➟ Feeling of fullness or pressure in the ear

➟ Dizziness or vertigo may occur

🟣 Signs Suggesting Infection

➟ Fever

➟ Ear discharge that is yellow, green, or foul-smelling

➟ Increasing ear pain

➟ Reduced hearing

Neck stiffness means reduced or painful movement of the neck, often with tight muscles or soreness. It is commonly caused by muscle strain or poor posture, but sometimes it can be a sign of a more serious condition.

🟣 Common Causes

➟ Muscle strain from poor posture, prolonged screen use, or awkward sleeping position

➟ Sudden neck movement or minor injury

➟ Cervical spondylosis or age-related wear of the neck joints

➟ Herniated disc or nerve irritation

➟ Tension headache or migraine

➟ Viral illness or other infections

➟ Less commonly, meningitis or serious spinal problems

🟣 Common Signs and Symptoms

➟ Difficulty turning or bending the neck

➟ Tight or sore neck muscles

➟ Pain that may spread to the shoulders or upper back

Deviated Nasal Septum: Signs & Management

A deviated nasal septum means the thin wall separating the two sides of the nose is shifted away from the centre. Mild deviation is common and may cause no problems, but a significant deviation can block airflow and contribute to nasal or sinus symptoms.

🟣 Common Signs and Symptoms

➟ Difficulty breathing through one or both sides of the nose

➟ One nostril may feel more blocked than the other

➟ Persistent nasal congestion

➟ Mouth breathing, especially during sleep

➟ Reduced airflow during exercise or when lying down

➟ Symptoms may worsen during colds or allergies

🟣 Other Possible Signs

➟ Recurrent nosebleeds due to dryness and abnormal airflow

➟ Facial pressure or discomfort

➟ Headache may occur in some people, although it is not specific to septal deviation

➟ Noisy breathing during sleep

➟ Snoring

➟ Dry mouth from frequent mouth breathing

🟣 Common Causes

➟ The septum may be crooked from birth

➟ Injury to the nose during sports, falls, accidents, or fights

➟ Nasal trauma during childhood may become more noticeable as the nose grows

➟ Age-related changes can sometimes worsen an existing deviation

🟣 Can It Cause Sinus Problems?

➟ Significant deviation can narrow the nasal passages

➟ This may interfere with normal sinus drainage in some people

➟ Recurrent sinus congestion or infections may occur

➟ However, allergies and chronic sinus inflammation can cause similar symptoms and may exist together with a deviated septum

🟣 Diagnosis

➟ An ENT doctor examines the inside of the nose

➟ Nasal endoscopy may be used to assess the septum and deeper nasal passages

➟ CT scanning is not needed for every case but may be used when chronic sinus disease or another problem is suspected

🟣 Medical Management

➟ Medicines do not straighten the septum, but they can reduce associated swelling

➟ Saline nasal rinses can help reduce dryness and congestion

➟ Steroid nasal sprays may help if allergies or inflammation are also present

➟ Antihistamines may be useful for allergic symptoms

➟ Decongestant sprays should generally not be used for prolonged periods because they can cause rebound congestion

🟣 Surgical Treatment – Septoplasty

➟ Septoplasty is surgery used to straighten the deviated septum

➟ It may be considered when persistent blockage significantly affects breathing, sleep, or quality of life

➟ Surgery may also be considered with recurrent nosebleeds or selected sinus problems

➟ Septoplasty is usually performed through the nostrils without an external facial scar

🟣 When to See an ENT Specialist

➟ Persistent nasal blockage despite appropriate treatment

➟ Frequent nosebleeds

➟ Recurrent sinus infections

➟ Significant snoring or sleep disturbance

➟ Difficulty breathing through the nose affecting daily activities

➟ Symptoms following a significant nasal injury
