The patient had a low bridge. For enhancements over the bridge area of the nose, we use either Goretex, Irradiated Rib, or the patient’s own rib. In this case we decided to use goretex as the material of choice.

Gore-Tex can improve and define the radix, but there are physical limits to how much it can be augmented while maintaining a natural-looking result. Because the radix is not an empty space where you can simply keep adding material. The amount of Gore-Tex that can be used is constrained by the surrounding anatomy and soft tissues. The skin and underlying tissues can only stretch and accommodate a certain amount of augmentation. Too much can create excessive tension or an unnatural contour. The existing nasal bones, frontal bone, and upper nasal framework determine the foundation on which the implant sits.

So, the limit isn’t simply how much Gore-Tex can physically be placed, it is how much can be added while maintaining stable anatomy, adequate soft-tissue coverage, and a natural transition.

Our patient already had good tip projection. She had a small nasal hump. She requested for something with a gentle slope and a refined tip.

We created a smooth, gradual transition from the nasal bridge to the tip rather than a very high or sharply defined bridge. This was achieved with a goretex implant to camouflage the hump over the bridge. The goal is a nose that has subtle definition and a soft, continuous profile, while remaining harmonious with the patient’s facial shape.

A gentle slope doesn’t necessarily mean a low bridge. The surgeon can increase bridge height while maintaining a gradual, harmonious slope. The important distinction is that the ideal slope depends on the patient’s existing anatomy and facial proportions. What looks elegant on one face may look excessive on another.

Our patient had a bulbous tip and wide nasal bridge. We addressed these concerns and improved nasal height.

A bulbous tip can often be refined during rhinoplasty by addressing the tip cartilage, soft tissue, and overall tip support. Reshaping the lower lateral cartilages creates a narrower, more defined tip. Tip grafting and structural support adds definition and projection. Managing thick soft tissue/skin, although this is more limited because skin thickness cannot simply be removed without risking contour irregularities.

The goal is usually not to make the tip unnaturally small, but to create better definition while maintaining adequate structural support and a smooth transition from the bridge to the tip. For a very bulbous tip, the final degree of refinement is also limited by the existing cartilage anatomy and skin thickness.
