In line with the declaration of MalacaΓ±ang Palace through Memorandum No. 116 issued on Tuesday, March 31, 2026, The Health Information Management Department will observe the following schedule:

* April 1, 2026 - OPEN (8:00 am to 12:00 noon)
* April 2 to April 3, 2026 - CLOSED

Regular operations will resume on Monday, April 6, 2026 (8:00am to 5:00pm

Patient Inquiry Portal (Medical Records) Happy Holidays! Please be advised that the Health Information Management Department is currently closed for the holidays. You may still submit your inquiries at any time, and we will respond to them as soon as we can. Our office will reopen on January 5, 2026.

Maaari kayong magpadala ng inyong mga katanungan anumang oras at malugod namin itong tutugunan. Ang aming opisina ay muling magbubukas sa Enero 5, 2026.
