📌 PAALALA: Kung hindi kayo sasali sa OJT, Sports Activity, Off-Campus Activity, o NSTP Activity, o wala kayong kailangang Medical Certificate/Clearance, hindi na kailangang sagutan ang Pre-Participatory Medical Evaluation Form.

Para sa First-Year at Transferee Students, ang kailangang kumpletuhin lamang ay ang Routine Medical at Dental Examination Forms.
