Registering to your YAKAP Provider is now easier using the eGov PH app! 📱💚

Follow these simple steps to choose Taft Main Health Center as your YAKAP provider:

✅ STEP 1: Log in to your eGov PH account and make sure your PhilHealth details are updated. Tap “Add YAKAP Provider.”
✅ STEP 2: Click “Add YAKAP Provider” again when prompted.
✅ STEP 3: Select your location:
• Province: Surigao del Norte
• City/Municipality: Surigao City
Then tap Search and choose Taft Main Health Center
✅ STEP 4: Once registered, you can now visit our facility anytime you need medical care.

PhilHealth Regional Office – Caraga is proud to announce the upcoming PhilHealth YAKAP + GAMOT Run (Leg 2), scheduled on May 10, 2026 in Surigao City, Surigao del Norte. With the theme "Lunas na GAMOT, YAKAP sa Bawat Pilipino," this event aims to promote health and wellness while strengthening community engagement within the region.
The run features 6KM and 12KM categories, with assembly set at 4:00 AM. Registration is FREE, but please note that slots are limited and available exclusively for Surigao del Norte Mainland YAKAP registrants.

How to Register:
To secure a slot, eligible participants must complete the following steps:
1. Complete the Form:

Duly fill out the PhilHealth YAKAP + GAMOT Fun Run 2026 Registration Form.
2. Prepare Requirements:
Secure a signed Mutual Care Agreement (MCA) from your chosen YAKAP Clinic (hard/soft copy).
Participants must have undergone a First Patient Encounter (FPE).
Bring any government-issued ID.
3. Submit:
Bring the accomplished form and MCA to the LHIO Surigao, Brgy. Luna, Surigao City, Surigao del Norte, the official registration site.

Siguraduhing updated ang inyong Primary Care Clinic (PCC) selection para tuloy-tuloy ang benepisyo sa ilalim ng PhilHealth YAKAP.

Kung may First Patient Encounter (FPE) noong 2025, automatic na mare-renew ang inyong clinic para sa 2026 kung walang request for transfer.

Kung walang FPE, kinakailangan muling pumili ng inyong YAKAP Clinic sa pamamagitan ng:
• PhilHealth Member Portal
• eGovPH Super App
• Pagbisita sa pinakamalapit na LHIO

Para sa walk-in beneficiaries, maaaring pumili ng clinic gamit ang portal o app, sa tulong ng PCU verification.

👉 Piliin ang tamang Primary Care Clinic para sa mas maayos at tuloy-tuloy na serbisyong pangkalusugan.
