Pineal region tumor resection remains one of the most technically demanding neurosurgical procedures. The challenge lies in the limited surgical corridor, further complicated by dense vascular structures overlying the operative field.

In this case, a 14-year-old male presented with a pineal region tumor. He underwent a successful supracerebellar infratentorial approach, resulting in gross total resection (GTR).

The patient has been safely discharged and is on a steady path to recovery.

This 2 year-old male came in due to a visible mass noted at the bridge of the nose. Further work up revealed a nasoethmoidal encephalocoele (NEM); he underwent repair of NEM where we removed the herniated brain and covered the defect at the skull base. After 2 weeks , this little cute boy followed up with unremarkable recovery.

Nasoethmoidal encephalocoele is a rare congenital defect where a portion of brain herniate through a defect in the skull. It is usually seen as a visible mass at the bridge of the nose. Surgical treatment includes removal of the herniated brain and covering the skull defect.

Interesting finding.

We performed the first endoscopic transphenoidal pituitary surgery in a private hospital (set up) in Tagbilaran City, Bohol for a patient who complaint of headache and blurring of vision.

Biopsy confirmed to be a pituitary tuberculoma, an extremely rare condition, even in an area where tuberculosis is endemic.

Fortunately, it responds well to our anti-tuberculosis medications.
