📢 ATTENTION, TUPVIANS!

This is to inform all concerned students regarding the upcoming SIT Summer Deployment for S.Y. 2026–2027, scheduled to begin on **July 6, 2026**.

Please be guided by the schedule for the **submission of medical requirements** and **physical examination** at the **TUPV Clinic** for the SIT Summer Deployment.

Kindly ensure that all required documents are submitted ON your assigned schedule to avoid delays in processing.

📌 IMPORTANT REMINDERS:
• Strictly place all your requirements in a **long brown envelope** together with a **plastic envelope**.
• Arrange your documents in the required order.

✔️ Use a GREEN EXPANDING ENVELOPE (NOT neon green envelope, NOT green plastic envelope and NOT green expanding folder).
✔️ Write your name (Last name, First name MI.) and contact number at the BACK UPPER LEFT CORNER of the envelope
✔️ Arrange documents in this order:

Medical Certificate
Drug Test
CBC
Hepatitis Test
Chest X-ray (not the xray film)

Please make sure that all requirements are complete and properly organized before submission to avoid delays.

📢 Important Notice for Chem and CPE SIT Students 📢

Kindly read the following for your guidance.

‼️Operating hours: 8:00AM to 5:00PM only.
🗓 March 9 to 10, 2026

The University Clinic will accommodate a maximum of 50 students daily for the processing of SIT Medical Requirements. We encourage students to arrive early and prepare all necessary documents to ensure a smooth process.

Please arrange the following requirements in this order:
✅️ Medical Certificate (if done outside)
✅️ Physical Examination Form (if done outside)
✅ ️Drugtest
✅️ Complete Blood Count (CBC)
✅ ️Urinalysis
✅️ Fecalysis
✅️ HbsAg
✅️ X-ray

Bring both the original copy and a photocopy of each required medical document.

The Physical Exam and Medical Certificate will be provided by our Medical Officer. 🩺

For those unable to secure a priority number or the University Clinic is unavailable, you may opt to visit City Health, Government, or Private Doctors as an acceptable alternative. Kindly note that any associated costs will be at your own expense.
