To all incoming first-year students: Along with your laboratory results, please secure and submit a Medical Certificate from a physician stating that you are "fit to enroll" or "fit for schooling."

To all Incoming First-Year College Students of CAPSU Tapaz Satellite College:

Please be informed that upon submission of your laboratory results, the following requirements must be observed:

📌 All documents must be placed inside a long brown envelope.

📌 Attach one (1) recent 2×2 ID picture to the envelope.

Kindly ensure that all requirements are complete and properly arranged before submission to the University Clinic to facilitate the processing of your enrollment documents.

Your cooperation and compliance with these requirements are highly appreciated.
