Home Visit and Assessment of Bedridden Patients at Barangays San Vicente, Sto. Cristo & San Nicolas

Through these home visits, CHC I's nurses and midwives conducted comprehensive assessments of bedridden patients to evaluate their current health status, monitor ongoing medical conditions, and identify any immediate health concerns requiring intervention.

CHC I is committed to delivering accessible, and patient-centered healthcare, ensuring that no one is left behind, especially those who are unable to visit the health center. 💙🏥

PuroKalusugan 2026 was successfully conducted at Block 5, Barangay San Nicolas, bringing essential healthcare services closer to the residents. The activity was led by the City Health Center I (CHC I) Staff with the goal of promoting disease prevention, early detection, and improving access to quality primary healthcare services for the San Nicolas community.

The following health services were provided to the residents of Barangay San Nicolas:

• National Immunization Program
• Maternal Health and Family Planning Services
• Nutrition Program Services
• Non-Communicable Disease (NCD) Risk Assessment
• Other Health Services
• PhilHealth Profiling and YAKAP Registration

Through PuroKalusugan 2026, CHC I remains committed to ensuring that every resident of Barangay San Nicolas has access to comprehensive, accessible, and people-centered healthcare services, empowering the community to achieve better health and overall well-being.
