🧠 Frontal bossing & prominent supraorbital ridges
🖐🏻 Enlarged hands & feet / spade-like fingers
👅 Macroglossia
🗣️ Deep, husky voice
🤕 Headache
👁️But no Visual-field defect

🧩 Pseudoacromegaly
🦋 Hypothyroidism
🦴 Paget disease
🧬 Familial/constitutional large hands & feet

🔹 Initial: 🧪 → Serum IGF-1

🔹 Confirmatory: 🥤 → 75-g OGTT → ❌ Failure of GH
suppression

🔹 Localization: 🧠 → Contrast-enhanced MRI of pituitary

🔹 Assess complications:
🩸 HbA1c / glucose
❤️ BP, ECG ± echocardiography
😴 Sleep study
👁️ Visual-field assessment
🧪 Other pituitary hormones

🩺🎯 4. Definitive Treatment
🥇 Transsphenoidal resection of pituitary adenoma
If persistent disease / unsuitable for surgery:

💊 Octreotide / Lanreotide

🕷️ Spider angioma / Spider nevus / Spider telangiectasia

🪽 It is a central dilated arteriole with multiple small radiating vessels, giving it a characteristic spider-like appearance.

🪽Typical features:
🔅Central red vascular point
Radiating vessels ("legs")
🔅Blanches on pressure (especially over the central point)
🔅Refills from the center when pressure is released

🪽Common associations

✨Chronic liver disease/cirrhosis — especially with hyperestrogenism
✨Pregnancy
✨Estrogen therapy/OCPs
✨Healthy individuals, particularly children and young women

Clinical pearl: Multiple spider angiomas, particularly >2–3, should prompt consideration of chronic liver disease, especially when accompanied by other stigmata of CLD.

🪽🪽Encountered a very interesting case of dysphagia today.

A young man of average height and build presented with a few weeks "history of difficulty swallowing solids".
Clinical examination was nearly unremarkable .He had already undergone investigations, which revealed the classic features of "Plummer–Vinson syndrome". 🔺
Plummer–Vinson syndrome (PVS) is also known as:

✨Paterson–Brown-Kelly syndrome
✨Plummer–Vinson disease
✨Sideropenic dysphagia
✨Iron-deficiency dysphagia

The classic triad is
dysphagia + iron-deficiency anemia + upper esophageal (post-cricoid) web. 🔺

PVS is extremely rare in the general population, but in Pakistan it may be found in roughly 5–11% of patients presenting with dysphagia, based on available hospital-based studies. 📊📈

🪽Iron deficiency is thought to contribute to:..Atrophy of the esophageal/pharyngeal mucosa..Impaired swallowing muscle function..Formation of the characteristic post-cricoid esophageal web 🔅

Correcting iron deficiency can reverse some of these changes, and improvement in dysphagia has been documented even with improvement of the web itself. ♻️

In a patient with confirmed PVS, treat the iron deficiency in all cases, while assessing the severity of the web/dysphagia to decide whether dilation is needed.🩺✔️

Also, the cause of iron deficiency should be investigated (GI blood loss, menstrual blood loss, malabsorption such as celiac disease, nutritional deficiency, etc.)‼️

Plummer–Vinson dysphagia may respond to iron therapy alone, but persistent/severe dysphagia requires endoscopic web dilatation. 💠
