🔸 Thick hyperkeratotic / verrucous nodules
🔸 Predominant shin involvement
🔸 Post-inflammatory hyperpigmentation & excoriations

🩺 Chronic itchy verrucous plaques on the legs should always be evaluated carefully. Dermoscopy and sometimes biopsy are essential to rule out mimickers and chronic inflammatory dermatoses.

Not every scaly scalp is simple dandruff.
Tinea capitis is a fungal infection of the scalp that can lead to:

⚠️ Hair loss
⚠️ Broken hairs (“black dots”)
⚠️ Itching & scaling
⚠️ Painful swollen patches (kerion)
⚠️ Permanent scarring if ignored

👦 Commonly seen in children, but adults can get it too.

📌 Clues on examination & dermoscopy may include:
• Black dots
• Comma hairs
• Corkscrew hairs
• Scaling around follicles

❌ Hair oils and steroid creams may worsen the condition.
✅ Proper oral antifungal treatment is usually needed.

Early diagnosis = better hair recovery.

Many skin rashes look similar at first glance… but in dermatology, the scale often gives away the diagnosis.

This picture shows multiple small scaly papules/plaques — and the differential changes completely depending on the type and pattern of scaling.

📌 Why is examining the scale so important?

Because the same-looking rash can represent different conditions such as:

🔸 Psoriasis
➡️ Thick, silvery-white adherent scale
➡️ Well-defined plaques
➡️ May show pinpoint bleeding after removing scale (Auspitz sign)

🔸 Pityriasis Rosea
➡️ Fine collarette scale (scale at the edge)
➡️ Often starts with a herald patch
➡️ Follows cleavage lines

🔸 Pityriasis Lichenoides Chronica (PLC)
➡️ Multiple small papules with fine “mica-like” scale
➡️ Chronic and recurrent course

🔸 Pityriasis Rubra Pilaris (PRP)
➡️ Follicular papules with scaling
➡️ May show orange-red plaques

These fine red lines are called telangiectasias — tiny superficial blood vessels that become visible on the skin. They are benign (non-cancerous), but because they appear on the face, they often become a cosmetic concern for patients and parents.

🔍 On dermoscopy, we can clearly see the typical fine branching/serpentine vessels, which helps confirm the diagnosis and rule out other vascular lesions.

💡 The good news:
Most cases are harmless, but evaluation is important to identify the exact type and decide if treatment is required.

📌 When should you consult a dermatologist?
✔ If the patch is increasing in size
✔ If there is associated redness, swelling, or bleeding

These can look like a simple “boil” or “wart”, but they may actually be a skin cancer that grows quickly.

🔍 Warning Signs to Watch For:
⚠️ A rapidly growing lump (weeks to months)
⚠️ A hard, rough, scaly patch
⚠️ A sore that doesn’t heal
⚠️ A growth with a central crust or keratin plug
⚠️ Bleeding or pain on touch

📍 Common Sites

☀️ Hands and arms
☀️ Scalp
☀️ Sometimes back or legs
