Case Spotlight: Delayed Intertrochanteric Femur Fracture Managed with DHS Fixation

A patient presented 1.5 months after sustaining a fall resulting in persistent pain around the hip, inability to bear weight, and significant limitation of daily activities. Due to delayed presentation and limited access to definitive treatment, the fracture remained untreated during the initial weeks following injury.

Clinical Evaluation
On examination, the affected lower limb was painful on movement with restricted hip range of motion. The patient was unable to ambulate independently and had tenderness around the trochanteric region. Neurovascular status of the limb was intact.

Investigations
• Plain radiographs of the pelvis with both hips and the affected femur revealed an intertrochanteric fracture of the proximal femur with signs of early callus formation due to delayed presentation.
• Routine pre-operative laboratory investigations including complete blood count, renal function tests, serum electrolytes, coagulation profile, blood grouping, and viral screening were performed.
• Anaesthetic fitness was obtained after comprehensive medical evaluation.

Surgical Management
Considering the patient's functional demands and the need for stable fixation, surgical management with a Dynamic Hip Screw (DHS) was planned. Intra-operatively, the delayed nature of the fracture posed challenges in achieving satisfactory reduction because of early healing and fibrous tissue formation at the fracture site.

After careful reduction under fluoroscopic guidance, a guidewire was placed in the optimal position within the femoral head. Reaming was performed followed by insertion of the lag screw and side plate assembly. Fracture alignment and implant position were confirmed on image intensifier views before wound closure.

Post-Operative Plan
• Pain control and thromboprophylaxis as indicated.
• Early mobilisation with physiotherapy.
• Gradual progression of weight bearing according to fracture stability and radiological progress.
• Regular follow-up with serial radiographs to monitor union.

Outcome
Stable fixation was achieved despite the 1.5-month delay from injury. The patient tolerated the procedure well and was mobilised post-operatively. Continued follow-up is planned to assess fracture union and restoration of function.

Delayed presentation increases the complexity of fracture management, but meticulous planning and appropriate surgical intervention can still provide excellent outcomes.

Another successful surgical management of a shaft of femur fracture with interlocking intramedullary nailing. This technique is considered the gold standard for most femoral shaft fractures, providing excellent stability while preserving the biology of fracture healing.

The goals of surgery are to restore the length, alignment, and rotation of the femur, allowing early mobilization and a quicker return to daily activities. Interlocking nails offer strong fixation, minimize soft tissue disruption, and promote reliable bone healing.

Another successful Carpal Tunnel Release surgery performed today.

The patient had been suffering from numbness, tingling, pain, and weakness in the hand due to compression of the median nerve at the wrist. Through a small surgical procedure, the pressure on the nerve was relieved, giving the patient the best chance for gradual recovery of sensation, improved hand function, and pain relief.

Early diagnosis and timely treatment can prevent permanent nerve damage and help patients regain a better quality of life.

A 4-year-old child presented with a displaced supracondylar fracture of the humerus. Distal neurovascular status was intact on presentation. Due to non-functional C-arm at the moment, open reduction was performed through a medial approach and fixation achieved with crossed K-wires. Ulnar nerve was identified and protected posterior to the medial condyle throughout the procedure. Post-operatively, distal circulation and neurological status remained intact. الحمدللہ successful outcome.

🦴 Shaft of Tibia Fracture — Successfully Treated with DCP Plating

A patient presented with a shaft of tibia fracture sustained 5 days prior to surgery. After proper evaluation and pre-operative planning, Open Reduction and Internal Fixation (ORIF) was performed using an 8-hole Dynamic Compression Plate (DCP).

A successful DCP (Dynamic Compression Plate) fixation of Radius & Ulna was performed on an 18-year-old patient presenting with a displaced fracture of both forearm bones following trauma.

Interlocking Nail (ILN) for Comminuted Fracture, Right Shaft of Femur:

A challenging case of comminuted fracture of the right femoral shaft, presented 18 days post-injury. Delay was due to logistical and resource constraints. Despite limited availability of implants, definitive fixation was successfully performed.
Intraoperatively, only one 9mm × 38cm interlocking nail was available. Careful preoperative planning, meticulous canal preparation, and precise technique were essential to achieve optimal alignment, length restoration, and rotational stability.
Even in resource-limited settings, principles remain the same:
✔️ Respect biology
✔️ Preserve soft tissue
✔️ Achieve stable fixation
✔️ Restore limb alignment
Open reduction and internal fixation was achieved as far as possible, and interlocking fixation provided satisfactory stabilization despite comminution. Cerclage wiring done. Postoperative radiographs confirmed acceptable alignment and implant position.
This case highlights that orthopedic surgery is not only about advanced setups, it is about decision making, adaptability, and commitment to patient care.

Outcome: Stable fixation, limb alignment maintained, patient mobilization protocol initiated.

A patient presented with an intertrochanteric fracture of the femur following trauma. After thorough evaluation and pre-operative optimization, fixation was performed using a Dynamic Hip Screw (DHS).

The procedure was carried out under image guidance with careful fracture reduction and stable implant placement. Intra-operative parameters were satisfactory, and alignment was confirmed.

The goal remains early mobilization, pain relief, and restoration of pre-injury functional status.
