Ingrown toenails (also known as onychocryptosis) refers to a condition where the side of a toenail grows into the surrounding soft skin around the nail fold. This growth causes pain, swelling, and tenderness in the corner of the nail. The most commonly affected part is the big toe, but ingrown toenails can develop on any foot. An ingrown toenail can reoccur if not treated properly.

𝐂𝐚𝐮𝐬𝐞𝐬 𝐨𝐟 𝐈𝐧𝐠𝐫𝐨𝐰𝐧 𝐓𝐨𝐞𝐧𝐚𝐢𝐥:

Factors predisposing a person towards developing ingrown toenails include wearing tight or ill-fitted footwear, trimming of the toenails either by cutting the nails too short or leaving the edges sharp, injury to the toenail, naturally curved toenails, genetic susceptibility, nail plate trauma which usually occur by dropping objects on the toenail or stubbing the toenail. Other causes can include using feet extensively during athletic activities (ballet, football, kickboxing ) in which repeatedly kicking of object or pressure on feet for long periods is involved can cause protrusion of the toenail which can lead to the injury of surrounding flesh resulting in an ingrown nail. Abnormally shaped nail bed, nail deformities caused by diseases or genetic susceptibility and improper foot hygiene such as not keeping feet clean or dry are other factors.

𝐑𝐢𝐬𝐤 𝐅𝐚𝐜𝐭𝐨𝐫𝐬 𝐨𝐟 𝐈𝐧𝐠𝐫𝐨𝐰𝐧 𝐓𝐨𝐞𝐧𝐚𝐢𝐥:

Risk factors that may predispose the person to develop an ingrown nail are obesity, diabetes, kidney, thyroid and heart problems (which may cause foot and leg swelling), onychomycosis i.e fungal infection of nails, bony or soft tissue tumors of toes, hyperhidrosis (excessive sweating of feet), too small or too large shoe gear, abnormal gait and poor foot mechanics such as pronation, toe or foot deformities such as bunions, hammertoes and flat foot, congenital toenail deformity abnormally long toes.

𝐒𝐢𝐠𝐧𝐬 𝐚𝐧𝐝 𝐒𝐲𝐦𝐩𝐭𝐨𝐦𝐬 𝐨𝐟 𝐈𝐧𝐠𝐫𝐨𝐰𝐧 𝐓𝐨𝐞𝐧𝐚𝐢𝐥:
Surrounding skin of the affected nail is painful, swollen and red and the affected part is usually tender. An infection may develop in the tissue surrounding the nail, leading to yellow or white pus discharge. The big toe gets affected in a majority of cases, either on one or both the feet.
Overgrowth of skin around the toe is common.

𝐂𝐨𝐦𝐩𝐥𝐢𝐜𝐚𝐭𝐢𝐨𝐧𝐬 𝐨𝐟 𝐈𝐧𝐠𝐫𝐨𝐰𝐧 𝐓𝐨𝐞𝐧𝐚𝐢𝐥:

An untreated ingrown nail can cause a nail infection which can further progress to a foot ulcer, open sores, loss of blood flow to the infected area, scarring of surrounding skin and nail borders.
An ingrown nail can also progress to tissue decay and tissue death at the site of infection (cellulitis).
Diabetes can complicate ingrown nail as even small cut, scrape may quickly become infected due to lack of blood flow and nerve sensitivity.
Toenail infection due to the ingrown nail can progress to the bone causing infection (osteomyelitis).
Lichen planus can also be one of the complications for ingrown nail as in about 10 percent of cases it causes ridging, grooving, splitting and thinning of nails.

Diagnosis of ingrown nail requires taking into account all possible causative factors and medical history.
An ingrown toenail can also be diagnosed with X-ray to show how deep the nail has grown into the skin.
X-Ray can also identify ingrown nail which was caused by injury.

𝑪𝒂𝒔𝒆 𝑺𝒖𝒎𝒎𝒂𝒓𝒚:

A 52-year-old female presented with advanced pancreatic carcinoma marked by severe epigastric pain radiating to the back, anorexia, marked weight loss, profound fatigue, and disturbed sleep. Her constitution was chilly, thirst was low, and anxiety centered on health and her husband’s concurrent illness. She approached me via email.

After briefly analyzing the whole case :

𝑴𝒚 𝑪𝒐𝒎𝒎𝒆𝒏𝒕 𝒂𝒏𝒅 𝑨𝒏𝒂𝒍𝒚𝒔𝒊𝒔:

Phase 1: Functional Detoxification and Vital Force Rebalancing:

The initial goal was to remove blockages in reactivity and prepare the organism for deeper remedies.
Nux Vomica 30C was prescribed nightly to relieve digestive congestion and accumulated medicinal or dietary toxicity, especially in patients with long-standing stress, irritability, and disturbed sleep. It acted on the gastric-liver axis to clear “vital stagnation.”

Sulphur 30C was given each morning to follow Nux Vomica, functioning as a deep antipsoric cleanser and constitutional stimulant. It enhanced the general reactivity of the organism, improved circulation and metabolism, and reopened natural eliminative channels. This phase successfully improved appetite, bowel regularity, and sleep, confirming the reawakening of the vital force.

Phase 2: Miasmatic Clearing and Genetic Axis Regulation:

The second phase targeted the deep-seated hereditary and acquired miasmatic influences underlying the malignant process.

X-Ray 30C was administered as an energetic neutralizer to counteract the iatrogenic and radiation effects from diagnostic exposure. It helped balance the disrupted cellular field before higher nosodes were introduced.

Carcinosin CM addressed the inherited cancer diathesis, suppressed emotions, and chronic anxiety. It worked on the syphilitic and sycotic planes simultaneously, restoring balance to the genetic cancer blueprint.

Tuberculinum Koch CM was introduced to correct the tubercular miasm associated with rapid wasting, nervous exhaustion, and poor assimilation. It stimulated vitality and improved appetite.

Calcarea Arsenicosa 200C was used twice daily, it's a principle remedy for Pancreatic Cancer, it has both features of Calcarea Carb and Arsenicum Album. It acts as a deep anti-syphilitic remedy to rebuild tissue resistance, check malignant degeneration, and combat cachexia in patients with cold extremities, fatigue, and gradual wasting.

This phase consolidated the earlier gains appetite normalized, weight increased by six pounds, pain resolved, bowel regularity returned, and energy improved. Sleep became intermittent mainly due to nocturia rather than disease-related discomfort.

𝑭𝒐𝒍𝒍𝒐𝒘-𝑼𝒑 𝑬𝒗𝒂𝒍𝒖𝒂𝒕𝒊𝒐𝒏:
The Follow-Up Evaluation was gauge by:
1) Hering’s Law of Cure
2) 12th Observations by J.T Kent.

Old destructive symptoms disappeared first, replaced by mild, self-limiting urinary irritation an indication of genuine healing rather than suppression.

According to Kent’s 12 Observations, the case corresponded to Observation 2 steady improvement without new symptoms. The remedy sequence remained curative in action, the potencies were appropriate, and the patient’s vitality continued to rise.

𝑪𝒍𝒊𝒏𝒊𝒄𝒂𝒍 𝑶𝒖𝒕𝒄𝒐𝒎𝒆 𝒂𝒏𝒅 𝑷𝒓𝒐𝒈𝒏𝒐𝒔𝒊𝒔:

The treatment produced authentic constitutional healing. Vitality, appetite, weight, and mood improved substantially, with no recurrence of core pathology or pain. The organism displayed a clear centripetal-to-peripheral healing direction, consistent with classical curative laws.

Congestive Heart Failure in a Dog:-)

Diagnosis:
Congestive Heart Failure secondary to congenital mitral valve insufficiency, with acute episodes of coughing, dyspnea, cyanosis, and cardiac enlargement.

Totality of Symptoms:
Clingy, anxious, and comfort-seeking during coughing fits; proud and dominant with other animals; worse from temperature extremes, after meals, and during evening and morning hours; cyanosis during episodes; alternating overpowering sleepiness and distress.

Prescription:

Carbo Vegetabilis 200c
One dose on weekly basis

Phosphorus 30c
Twice daily

Remedy Justification:
Carbo Veg 200C was selected for advanced cardiac and respiratory weakness with cyanosis, venous stasis, and collapse tendency. It improved oxygenation and cardiac tone.

Post-Traumatic Hypertension Restored to Normal

Patient Summary:
A female patient developed persistent hypertension following trauma and orthopedic surgery. Before the injury, her blood pressure was consistently low (~98/64 mmHg). After the fall on June 18th, it remained elevated (~143/100 mmHg) and unresponsive to initial self-prescribed remedies.

Patient has message me with these words:

“Dr. Ikram Manzoor, please help. What would you try for hypertension after injury that won’t go down now? It spiked the day of the fall. No BP issues before. Injury happened June 18th.
I took Aconite (with Arnica/Bellis perennis) the same day and continued before and after surgery on July 1. Surgery was done on the right arm to place a plate and pins three fractures. I also took Symphytum and Calc Phos.”

My Comments on this Case:

O***m 200c: One dose only
Rationale:
O***m is indicated in conditions where the vital reaction is suppressed after shock or trauma. It restores normal reactivity when the system becomes sluggish or overexcited post-injury or anesthesia. The patient's persistent hypertension following surgical shock pointed toward a nervous system imbalance and vasomotor dysregulation, classically covered by O***m. In my own understanding it's actually works when something becomes stucked or sluggish, i.e in this case persistent fear was dominant.

Arnica CM: One dose only
Rationale:
Arnica acts on traumatic injury, tissue bruising, and surgical trauma. It supports capillary repair, improves local circulation, and alleviates post-operative inflammatory strain. In this case, it addressed the underlying trauma to soft tissues and residual vascular irritability contributing to raised BP.

Hypericum CM: One dose only (3 hours later)
Rationale:
Hypericum targets nerve injuries and post-surgical neuralgic sensitivity. It complements Arnica when nerve shock or pain persists after trauma or operation. It helped modulate nerve excitability, preventing sympathetic overactivity responsible for sustained hypertension.

Ruta CM: One dose only (3 hours later)
Rationale:
Ruta acts on periosteum, tendons, and connective tissues, especially after bone injury or surgical fixation. It accelerates healing in cases involving plates, pins, and fractures, as in this patient’s right arm. Ruta also relieves residual soreness and stiffness, indirectly reducing systemic stress influencing BP.

Rationale:

Stargardt disease (also called Stargardt macular dystrophy or juvenile macular degeneration) is a hereditary retinal disorder that leads to progressive loss of central vision, which was resolved within 3. months.

Patient’s Summary:
A 9-year-old boy diagnosed with Stargardt disease, a rare hereditary retinal degeneration causing progressive central vision loss, was brought for homeopathic treatment. Diagnosis was confirmed by an ophthalmology panel at CMH Rawalpindi, Pakistan through clinical evaluation and OCT imaging, which showed outer retinal thinning and photoreceptor layer loss.

The patient’s symptoms included:

Gradual central vision loss uncorrected by glasses

Central blind spots (scotomas)

Distorted or blurred central vision (metamorphopsia)

Difficulty in reading and recognizing faces

My Comment and Analysis:

Nux Vomica was used to clear iatrogenic and dietary toxins, optimizing systemic elimination and improving remedy receptivity. It rebalances the hepato-gastric axis, which is often sluggish in children exposed to medications or environmental stressors.

Sulphur acts as a constitutional cleanser, stimulating metabolic and eliminative organs. It strengthens reactivity and clears suppressed miasmatic layers, preparing the system for deep-acting nosodes.

X-Ray 30C: Initiates cellular-level stimulation and energetic restructuring of damaged retinal tissues. Useful in hereditary and radiation-linked degenerations.

Carcinosin CM: Addresses inherited diathesis and genetic predisposition, particularly in degenerative diseases with suppressed vitality and strong family tendencies.
