A 24-year-old lady who is 22 weeks pregnant (primigravida) is referred by gynaecologist for assessment of palpitations. She describes episodes of tachycardia since teen ages and has noted her rate jumping to 180 at times. She has used Tabs. Propranolol of and on with variable response.

Physical Examination is unremarkable.
Cardiac ausultation normal.
Heart rate 150 bpm which responded to carotid massage transiently.

Thyroid profile is normal
CBC: Hemoglobin 11.5

What's the abnormality on ECG?
How should she be managed?

A 45-year-old lady is experiencing episodes of near fainting and giddiness. Clinical examination is unremarkable.
