Pakistani women have the highest death rate from heart disease among women in five South Asian countries, according to a 2026 study published in JACC: Asia.

The study, based on Global Burden of Disease estimates, compared Pakistan, India, Bangladesh, Nepal and Bhutan. It found that in 2021, Pakistan had the highest age-standardised death rate from heart disease among women. Pakistan was also the only country where the estimated death-to-case ratio was higher for women than men — 4.4% compared with 3.1%.

WHO data from 2021 puts Pakistan’s estimated death rate from heart disease among women at 90.9 per 100,000, making it the leading reported cause of death among Pakistani women.

Doctors say high blood pressure, diabetes, obesity, physical inactivity and diets low in vegetables and fibre are among the factors linked to the risk. They also pointed to gaps in screening and follow-up, particularly during pregnancy, after childbirth and after menopause.

At Jinnah Postgraduate Medical Centre, doctors said heart disease was found in around 4% of pregnant women screened, while more than 90% of women attending its diabetes clinic had at least one cardiovascular risk factor. These figures are from specific hospital groups and do not represent the wider female population.

Doctors also raised concerns about gaps in primary and secondary cardiac care, including patients stopping prescribed medicines after procedures such as angioplasty and the pressure on specialised hospitals treating large numbers of heart patients.
