Melatonin regulates circadian rhythm and was proposed to reduce delirium by improving sleep–wake patterns. A placebo-controlled RCT meta-analysis found no significant effect on delirium incidence or mortality across 6 high-quality trials (2209 patients) and 13 trials (2830 patients), including the DEMEL trial with 0.3 mg nightly.
