Grief and drinking are connected far more often than people realise, and almost no one arrives naming the first one.

People come in for the drinking. Or the burnout. Or the exhaustion, or the sense that they have not felt like themselves in a long time. They point at the thing that is easiest to point at.

Sometimes, underneath the thing they came in for, is a grief that was never given anywhere to go. A loss from years ago that got managed instead of felt, because there was no time to stop, or no space, or no permission to fall apart. The evening glass of w**e that began as a way to switch off was never really about the w**e. It was how the grief kept getting through each day without being named.

This is not true for everyone, and it is not a formula. But it is common enough that a complete assessment does not take the presenting problem at face value. It asks, gently, what is underneath it, and gives that somewhere to go.

Why you cannot sleep in your forties is rarely just about sleep, and treating it as if it is may be the reason nothing has worked.

You are so tired but you cannot sleep. You take something to get through the night. You wake up tired anyway. The next night it starts again. You mentioned it to your GP and were told it was stress.

It might be. It might also be several things at once, none of which a sleeping tablet reaches.

Sleep is one of the first things to break when hormones shift, when mood drops, when the evening glass of w**e became a nightly one, when a prescription that was meant to help started to feel necessary, or when a grief was never given anywhere to go. Often more than one of these is true at the same time.

That is why the honest question is not how to make you sleep. It is what is keeping your nervous system from feeling safe enough to rest.

There is not one clean name for it in standard mental health vocabulary. Which is part of why the women living inside it stay quiet about it for so long.

The life is on paper what was wanted. The partnership is intact. The children are thriving. The career is where it was supposed to be at forty-two. And underneath all of it, quietly, is a woman who has been running on cortisol and coping for years, who has stopped feeling like herself in a way she cannot explain to anyone without it sounding like ingratitude.
This is not a character flaw. It is not a phase. It is not something more sleep or a good holiday will fix. It is what happens to a nervous system that has been holding a beautiful, demanding life together without anyone assessing what the holding is actually costing.

At Algarve Wellness there is one clinical team held together across the residency from admission through to discharge. Aftercare includes a dedicated recovery coach who can be virtual or based in the client’s home country, and who can collaborate with the client’s existing psychiatrist. Continued care is also available through the partner programme in Greece.

None of that will appear as a decorative feature on the front page. It is the actual structure. It is also the difference between a residency that produces a short-term reset and one that produces a change the client is still living inside at month six.

Integrated mental health assessment for women who have been sent from one specialist to another for years without anyone looking at the whole picture.

She has been to her GP about the exhaustion. She has been to a therapist about the mood. She has quietly wondered whether she is drinking more than she should, and she has not mentioned it to either. Somewhere in her life there is also a hormonal picture that has never been properly assessed, because the presentations were treated as separate problems by different clinicians who do not speak to each other.

The residency is 30 days, sometimes longer, sometimes shorter, set with the client at the start. What decides whether the residency held is not what happens in those days. It is what happens at month three, when the client is back in the house, the relationships, and the pressures that were part of the original picture.
