“Don’t bother treating it… It will just come back.”

If you have heard this before, whether from a friend, online or even during a consultation, I understand why it can feel discouraging. Melasma is often misunderstood as hopeless, but in reality, it can be managed effectively with the right approach.

I have had patients sit across from me and say:

“Doctor, I was told there is no point treating melasma, because I will come back again, so I have just been leaving it alone.”

But over time, they start to notice something else.

It gradually becomes more noticeable, and darker.
More obvious in certain lighting or in photos.

And that is usually when they come in and ask, “Is there really nothing that can be done?”

The short answer is, yes we can do something about it!

Melasma is not untreatable. But it does need to be understood properly first by patients. It is considered a chronic skin condition [1], which means we are not looking for a one-time fix. Instead, we focus on controlling it well over time.

Sun spots and PIH may look similar at first glance, but they behave very differently beneath the surface.

If you’ve ever looked in the mirror and thought, “Why are these dark spots not fading no matter what I use?”

Well, you’re not alone. I see this all the time in the clinic.

A patient comes in, tells me they’ve been diligently applying brightening creams for months… sometimes even trying a laser elsewhere… but the spots are still there. Or worse, they’ve become darker or more widespread.

Often, the issue isn’t effort. It’s that we’re treating the wrong type of pigmentation.

Not all dark spots are the same, even if they look similar at first glance. Two of the most common types I diagnose are sun spots and post-inflammatory hyperpigmentation (PIH), and confusing one for the other can lead to frustrating results.

But, understanding the difference is where things start to change.
