Can You Treat Pigmentation If You're Always Under The Sun?

Many people assume that treating pigmentation is pointless if they spend a lot of time outdoors. In reality, pigmentation can still be managed — but treatment expectations and strategies may need to be adjusted.

For conditions such as melasma, UV exposure is only one of several contributing factors. Heat, inflammation, genetics, hormonal influences, and melanocyte activity may also play a role.

Patients with highly active outdoor lifestyles may require:
• More consistent sun protection
• Closer maintenance and follow-up
• Additional support from skincare or medical therapies
• Lifestyle modifications where practical

The goal is not necessarily to eliminate all sun exposure, but to reduce unnecessary triggers while keeping pigmentation under better long-term control.

🎥 In this video, Dr Thean explains why outdoor activities do not automatically prevent successful pigmentation treatment — and how treatment plans can be adapted to different lifestyles.

What To Expect After HIFU Part 2

One of the most common misconceptions about HIFU is that the final lifting result appears immediately after treatment.

In reality, HIFU works by stimulating gradual collagen remodeling within deeper support structures of the face. This process takes time.

Typical treatment progression:
• Mild puffiness may be present during the first few weeks
• Some early lifting may occur due to tissue contraction and temporary swelling
• More noticeable improvements often develop after 1–3 months
• Results commonly peak around 3–4 months
• Effects may last 6–9 months or longer depending on individual factors

Because collagen production is a gradual biological process, patience is often an important part of achieving the full benefits of treatment.

🎥 In this video, Dr Chiam explains the typical timeline of HIFU results and what patients can realistically expect after treatment.

When Pico Lasers Don't Work, What Could Be The Reason?

Not all pigmentation behaves the same way, which is why the same laser treatment can produce very different results between patients.

Several factors can influence treatment outcomes:
• Different pigment conditions respond differently to lasers
• Pigment may be located in superficial, deep, or multiple skin layers
• Individual skin type and sensitivity affect treatment planning
• Conditions such as melasma often involve inflammation and other underlying triggers
• Multiple sessions are usually required for gradual pigment clearance

In many cases, the challenge is not the laser itself, but accurately identifying the type of pigmentation and selecting the most appropriate treatment strategy.

🎥 In this video, Dr Daniel discusses why some pigmentation responds well to pico lasers while other cases may require a more comprehensive approach.

What To Expect After HIFU Part 1

HIFU is a non-invasive lifting treatment that works by delivering focused ultrasound energy to deeper structural layers beneath the skin.

Because there are no incisions or significant surface injury, downtime is generally minimal. However, some temporary post-treatment effects can occur as the skin begins its natural healing process.

Common early effects may include:
• Mild redness in treated areas
• Temporary tingling or sensitivity
• Slight swelling or raised areas in thinner skin regions
• Mild aching or tenderness for several days

These effects are typically short-lived and generally do not interfere with normal daily activities.

🎥 In this video, Dr Chiam explains what patients may experience in the hours and days following a HIFU treatment.

Can Melasma Be Treated With Lasers? Part 2

Yes, lasers can play an important role in melasma treatment — but successful outcomes depend on much more than the laser alone.

Factors that can influence results include:
• The type of laser used and treatment settings
• Skin type and depth of pigmentation
• Underlying inflammation and skin sensitivity
• Supporting topical and oral treatments
• Sun protection and long-term maintenance

Because melasma is a chronic condition influenced by multiple factors, treatment often requires a comprehensive approach rather than relying on a single device or procedure. The goal is not only to reduce existing pigmentation, but also to improve skin stability, minimize ongoing triggers, and reduce the likelihood of recurrence over time.

🎥 In this video, Dr Thean explains why melasma treatment is often about managing the condition, not simply removing the pigment.

Can Pico Lasers Treat Acne Scars? Part 2

Effective acne scar treatment often depends on identifying the underlying cause of the scar rather than relying on a single device.

Different acne scars may involve:
• Scar tethering beneath the skin
• Collagen loss and dermal thinning
• Uneven skin texture and surface irregularities
• Post-inflammatory hyperpigmentation

Because of this, treatment often involves a combination approach. Techniques such as subcision, fractional resurfacing lasers, RF microneedling, and other collagen-stimulating treatments may be used to target different aspects of the scar.
Acne scar improvement is also gradual. New collagen formation and tissue remodeling occur over weeks to months, which is why meaningful results typically require multiple sessions and a structured treatment plan.

🎥 In this video, Dr Daniel explains why acne scar treatment is often customized according to scar type, depth, and skin characteristics rather than relying on a single technology.

Is Pico Laser Not Effective in Treating Melasma?

Melasma is a complex and chronic pigmentation condition that involves more than just excess pigment within the skin.
In addition to pigmentation, melasma may be influenced by:
• Overactive melanocytes
• Chronic inflammation
• Increased vascular activity
• Basement membrane damage

This is why multiple treatment sessions are often required, regardless of the device used. While pico lasers can help reduce existing pigment, long-term improvement often requires addressing the underlying factors that contribute to recurrence.

For many patients, treatment involves a combination approach aimed at reducing pigment while maintaining skin stability and minimizing inflammation.
🎥 In this video, Dr Daniel explains why melasma often requires more than a single treatment modality — and why multiple sessions do not mean the treatment is ineffective.

Can Pico Lasers Treat Acne Scars? Part 1

Pico lasers can be effective for certain skin concerns, but pitted acne scars are often more complex than a surface-level problem.

Pitted scars develop when acne damages the deeper structures of the skin, leading to:
• Collagen loss within the dermis
• Loss of structural support beneath the skin
• Fibrotic scar bands that tether the skin downward

While pico lasers can help improve pigmentation and stimulate some degree of collagen remodeling, they do not directly release scar tethering or significantly rebuild deeper structural support.

This is why some patients may notice improvement in skin tone after multiple pico laser sessions, but see only limited changes in the depth of their acne scars.

🎥 In this video, Dr Daniel explains why pitted acne scars often require a different treatment approach from pigmentation concerns.

Why Do I Still Get Acne as an Adult?

Acne is not just a teenage condition. Many adults continue to experience acne well into their 30s, 40s, and beyond. In Singapore, factors such as hormonal fluctuations, stress, poor sleep, excess oil production, and the hot, humid climate can all contribute to persistent breakouts.

Adult acne is often influenced by multiple factors:
• Hormonal changes and conditions such as PCOS
• Stress and poor sleep
• Genetics and excess oil production
• Environmental factors and inflammation
• Diet, lifestyle, and skincare habits

Because adult acne is often multifactorial, treatment goes beyond treating active pimples. Identifying triggers early and controlling inflammation can help reduce the risk of acne marks, pigmentation, and scarring.
🎥 In this video, Dr Chiam explains why acne can persist into adulthood and why adult acne often requires a more personalised treatment approach.

Can HIFU Cause Pigmentation?

HIFU delivers focused ultrasound energy to deeper layers of the skin while largely sparing the skin surface. As a result, pigmentation directly caused by HIFU is generally uncommon. In rare situations where significant inflammation or thermal injury occurs, post-inflammatory hyperpigmentation (PIH) may develop, particularly in individuals who are more prone to pigmentation.

It is also important to recognise that not all pigmentation appearing after treatment is caused by the treatment itself. Underlying pigment conditions such as melasma or sun-induced pigmentation may already be present and become more noticeable over time.
This is why proper assessment is important:
• Identifying the type of pigmentation present
• Determining whether inflammation or injury has occurred
• Distinguishing PIH from underlying pigment conditions

Management depends on the underlying diagnosis and may involve reducing pigment activity, controlling inflammation, and supporting long-term skin stability.

🎥 In this video, Dr Thean discusses whether HIFU can cause pigmentation and why accurate diagnosis is important.
