Join Prof. Ki Jeong Hong, Clinical Professor of Emergency Medicine at Seoul National University Hospital, as he shares insights from South Koreaโ€™s HEROES Projectโ€”an innovative community-based initiative designed to strengthen the chain of survival through dispatcher-assisted CPR education and targeted public engagement.

The HEROS (Home Education and Resuscitation Outcome Study) Project focuses on empowering ordinary citizens, particularly family members and home bystanders, with the confidence and skills to recognise cardiac arrest, collaborate effectively with emergency dispatchers, and initiate life-saving CPR before EMS arrival.

Research involving more than 10,000 OHCA patients demonstrated that implementation of the HEROS programme was associated with significant improvements in neurological outcomes among patients experiencing cardiac arrest in the community.

In cardiac arrest resuscitation, every intervention mattersโ€”but are we paying enough attention to ventilation?

Two recent large multicentre studies provide important insights into how ventilation and airway management influence outcomes following out-of-hospital cardiac arrest.

๐Ÿซ Bag-Valve-Mask Ventilation Matters

In a multicentre study involving nearly 2,000 OHCA patients, only 40% achieved effective lung inflation in at least 50% of ventilation pauses during CPR. Patients who achieved effective ventilation had significantly higher rates of:

โœ… Return of spontaneous circulation (40.7% vs 25.2%)
โœ… Survival to hospital discharge (13.5% vs 4.1%)
โœ… Favourable neurological outcomes (10.6% vs 2.4%)

๐Ÿซ Advanced Airway Strategy Matters Too

A study of 350,216 OHCA patients across 254 EMS agencies demonstrated increasing adoption of supraglottic airways (SGAs) over time. In EMS systems with lower baseline survival rates, transitioning from endotracheal intubation (ETI) to SGA use was associated with:

โœ… Improved ROSC (25.7% โ†’ 29.1%)
โœ… Improved survival to hospital discharge (5.6% โ†’ 6.3%)

The message is clear:

๐Ÿ”น Use the right airway at the right time
๐Ÿ”น Match the airway strategy to the patient, provider, and EMS system
