Patients diagnosed with atypical ductal hyperplasia, atypical lobular hyperplasia, or classic lobular carcinoma in situ ( ) should be counseled about elevated future risk and offered comprehensive risk assessment and risk-reducing strategies when appropriate.

These guidelines indicate that atypical lobular hyperplasia and classic lobular carcinoma in situ can be safely observed when radiographic-pathologic concordance is established, whereas most atypical ductal hyperplasia cases identified on core biopsy should undergo diagnostic excision.

Taken together, current evidence suggests that the role of RT after mastectomy should be individualised, particularly in patients with HER2-positive disease and low nodal burden. While advances in systemic therapies may reduce the absolute benefit of RT in some patients, its contribution to locoregional control remains clinically relevant in selected cases.
Our findings support a more nuanced approach to postmastectomy RT in HER2-positive breast cancer, highlighting the need for careful risk stratification and multidisciplinary decision-making. Further prospective studies are warranted to better define which patients may safely omit RT and which are most likely to benefit from its use.
