Pharmacy Benefit Managers (PBMs) are quietly raking in hidden profits through spread pricing and rebate collection. When these PBMs are vertically integrated, they manipulate their structure to shift and obscure funds within their own operations. They may claim they can’t reimburse their own pharmacies more than independent ones—a claim that’s highly questionable—but the real money is in spread pricing.

If insurance providers were prohibited from owning PBMs, they would have no choice but to negotiate fairly, ensuring that spread pricing and rebates benefit the insurance plan itself. This shift would easily cover reasonable dispensing fees for pharmacies while keeping the market competitive. PBMs would return to their rightful role—simply transferring funds rather than exploiting the system.

It’s time for real reform. Let’s put an end to these hidden profit schemes and build a fairer, more transparent system
