The amount of control it takes to not show how excited I am for my patients when they reach their goals is unbelievable.

I am often times with my patients at some of the most vulnerable times of their lives. I address symptoms that are difficult to talk about and have.

Let’s face it… life can be challenging and we could all use some nervous regulation strategies. Whether you have pelvic floor symptoms or not, creating balance in the body can help all body systems. This is one of those strategies that someone may not typically think of with pelvic PT, but I can assure you that it can be such a big game changer for so many pelvic floor conditions (ie bowel, bladder, and pain).

It may seem surprising to some patients when I start asking more information about their current and past bowel habits and diagnoses when their current symptoms are more directed towards the front end of the pelvic floor.

When we zoom out and really look at the pelvic floor it makes sense why we need all the information and not just part.

If you have current or past history of symptoms or diagnoses listed below make sure to keep an open mind of where your symptoms may be coming from.

- constipation
- straining to p**p
- incomplete bowel emptying
- hemorrhoids
- a**l fissures
-re**al pain
-f***l incontinence

What are some front end symptoms I have treated that have had some connection with the back pelvic floor muscles or bowel function?
- urinary incontinence
- overactive bladder, urinary urgency/frequency
- urine retention or incomplete emptying
- pelvic pain (vaginal, vulvar, pe**le, scrotal, urethral)
- sexual symptoms (pain with pe*******on, arousal pain, pain with climax or post climax)
- pelvic organ prolapse

What should you do if you have pelvic floor symptoms?
Seek a pelvic floor evaluation. Stay consistent with treatments and keep an open mind about what may help your symptoms.

Why one exercise doesn’t usually fix bladder leakage is because we are more complex that a cookie cutter treatment.

It comes down to a few basic things when we are first looking at the drivers of p*e leakage and will help guide us in the right direction.

✅ What are the muscles doing? Strength, endurance, coordination. Yes for the pelvic floor muscles exam, but also in real life what are they doing? Sensation and a person knowing what their muscles are doing is also informative.
✅ What is the muscle tone? Are the muscles super tight or super relaxed? We should be able to go from each range of tone but this can change the focus from pure strengthening to also more coordination training to relax too.
✅ Is there pain present? Pain must be treated. This can often be why muscles are acting differently now than they did in the past.
✅ Is there urge present? This focuses on getting to the root of the urgency, which could be a stimulus, stressors, reduced sleep, food/drink, bathroom environment, an upregulated nervous system, etc

The more information we know, the better when can treat and the better our outcomes can be.

Our home exercise programs are geared towards function. Yes, sometimes we focus on traditional exercises to optimize in daily function. We can break this down a ton depending on your symptoms. If you don’t understand why your PT gave you an exercise or something to do at home make sure to ask. All exercises program or modifications to your activities of daily living should always be geared towards your big goals.

Your body may still be trying to protect you from those big events, even though mentally you may have processed them and moved on.

You didn’t do anything wrong. You just need help to get out of that fight, flight, or freeze mode aka the sympathetic nervous system.

I’ve seen symptoms of all ranges such as:
➡️ urinary urgency and/or frequency with or without p*e leakage
➡️ constipation
➡️ diarrhea
➡️ slow urination stream
➡️ pelvic pain of all types
➡️ erectile dysfunction
➡️ abdominal pain
