A Patient Centered Medical Home is an approach to providing comprehensive primary care for children, youth, and adults. The PCMH is a healthcare setting that facilitates partnerships between individual patients, their personal healthcare providers, and when appropriate, the patient's family. The provisions of medical home allow better access to health care, increased satisfaction with care and imp

roved health. Family Medicine/ Osteopathic Medicine/ Internal Medicine/ Obstetrics/ Med-Peds/ Behavioral Health/ Dental Health/ and school-based healthcare services. In addition to this, we offer health education on most any health condition you may require.

What an incredible day at Project Connect in Cheboygan! ❤️

Yesterday, through a true community-wide effort, we were able to connect residents with 23 community resources, provide food assistance to 112 households, and offer a free meal featuring pizza generously donated by BC Pizza of Cheboygan and coffee courtesy of McDonald's of Cheboygan.

One of the highlights of the event was our free Parent-to-Parent Swap Table, where neighbors came together to share needed items including diapers, baby formula, backpacks, children's supplies, and clothing for adults.

This event would not have been possible without the support and collaboration of the Food Bank of Eastern Michigan, our dedicated community partners, and the Cheboygan County Fairgrounds for so generously hosting us again this year.

Thank you to everyone who donated, volunteered, partnered, and attended. Your generosity and commitment to helping others made this event a tremendous success. Together, we are building a stronger, more connected community! 💚

This Labor Day, we’re taking a moment to recognize and appreciate the hard work, dedication, and contributions of our community, our patients, and our incredible team. ❤️

We are going live with Self-Scheduling! 🌟

What does this mean?

Patients will now have the option to schedule their own appointments with their preferred provider without calling. This feature will be available right on the homepage of the website, and there will be social media posts with a tutorial showing patients how easy it is. This will be exclusively for sick/walk-in appointments and will schedule for 15- to 20-minute blocks.

Join us to have your senior get their picture taken for school, get a family photo, have a new headshot taken while you look for employment. 📸

Join us for the Parent-to-Parent Swap - bring a gently used/new item and take what you need. 🎒

We are excited to host another year of Project Connect for the community to introduce resources that cover Cheboygan County residents, provide a free lunch and coffee, and food to take home. 🍕
