Today I finished my last surgical case in a hospital. After nearly three decades in urology, I’m stepping away from hospital work and, best of all, hospital call. No more middle-of-the-night phone calls.

It’s a little bittersweet to close this chapter, but I’m excited for what comes next and for the chance to practice what I preach by taking better care of myself, too. I’m grateful for the patients, colleagues, nurses, and friends who have been part of this chapter of my life.

The PeeDoc isn’t retiring, just getting a little more sleep.

“Hi doctor, I think I’m in perimenopause.”

Doctor:
“Have you tried smiling more and taking an antidepressant?” 😵‍💫

Meanwhile she’s 38, waking up at 3am, sweating through pajamas, forgetting why she walked into rooms, crying at dog food commercials, and wondering where her libido went.

Not every woman needs HRT.
Not every symptom is hormonal.

But a whole generation of women got diagnosed with “stress” while perimenopause sat in the corner waving frantically.

Doctors in 2002:
“Women after menopause need weight-bearing exercise to protect bone health.”

Also doctors in 2002:
“Definitely don’t take hormones.”

So women were basically sent outside to dig their own graves for osteoporosis prevention. 🪦💀

Good news: medicine has moved on since then.

A woman’s brain eats itself during menopause…”

If it wasn’t so frustrating—and honestly, so common—you’d think that was the setup to a bad joke.

But for a lot of women, it feels pretty real some days.

What’s really happening?
Estrogen plays a huge role in brain function. When it drops, your brain has to adapt and that’s where the brain fog, memory lapses, and mood changes show up.

Here’s the part most people don’t hear:
When used appropriately, hormone therapy may help support brain function and reduce long-term cognitive risk.

So no, your brain isn’t eating itself. But it is asking for some help.

Marge can’t get help for perimenopause. Her doctor shrugs and basically says, “just laugh it off.”

Meanwhile? Homer’s ED gets treated like a five-alarm emergency.

Sound familiar?

This is exactly how women’s health has been handled for years — underdiagnosed, dismissed, and misunderstood… especially in midlife.

Perimenopause isn’t a joke. It’s not “in your head.” And it’s definitely not something to just power through.

The good news? The conversation is changing. The science is catching up. And women are finally getting the attention they deserve.

About time.

Here’s one that deserves more attention - your actual heart.

I appreciated this message from , because it highlights something we don’t talk about enough:
Progesterone, estradiol, and testosterone are not just “reproductive” hormones. They are deeply involved in cardiovascular protection.

They help maintain healthy blood vessel function.
They support cholesterol balance.
They reduce inflammation.
They improve glucose regulation.

Then perimenopause begins. Hormones fluctuate. Eventually they decline. And with that loss, blood pressure rises, lipid profiles shift, insulin resistance increases and cardiac risk accelerates.

Too many women are never told that these changes are directly connected to hormone depletion.

Understanding that connection creates opportunity. Not fear, clarity. Earlier intervention. Smarter conversations about risk reduction.

Heart disease is still the leading cause of death in women. But it is not inevitable.

When hormones are part of the discussion, women gain agency — and a better chance at protecting their hearts long term.

💥 A Historic Day for Women’s Health
Today marks a turning point. The FDA has finally agreed to remove the outdated black box warnings linking estrogen therapy to breast cancer, heart disease, and dementia.
This is huge. For decades, fear and misinformation kept millions of women from safe, effective treatment. But the data has always told a different story—when used correctly, estrogen therapy protects brain, bone, and heart health.

Massive respect to the leaders who helped make this happen—Dr. Laura Jackson, Dr. Casey Means, Dr. Rachel Rubin, and Dr. Kelly Casperson—for bringing truth, science, and compassion to the FDA. And a sincere thank-you to Dr. Makary and all who have worked behind the scenes to move women’s health forward.

This is what progress looks like.

Science evolves—thankfully.
We once said “don’t eat eggs” … then “okay, just egg whites” … now “eggs are fine.”
Same with estrogen. We used to fear it—now we know it protects heart, brain & bones.
Learning is progress.
