I walked into an independent pharmacy today not because it was trendy, not because it had a flashy app, but because it was there.

Before I even made it to the counter, someone looked up and said my name. Not because I scanned a QR code. Because they knew me.

They asked how I was doing and then paused long enough to actually hear the answer.

Behind the counter wasn’t a script or a call center three states away. It was a pharmacist who knew my history, my meds, my allergies, and the fact that the last time I was in, I wasn’t doing so great. They remembered. And that mattered more than any coupon ever could.

They caught a dosing issue before it became a problem. They explained a side effect I may experience. They asked if I could afford the medication and when I hesitated, they quietly worked to find a solution without making me feel small.

No algorithm did that.
No insurance company approved that moment.
No big data model predicted that care.

Big insurances and the PBMs they own can’t see this.

They don’t see the pharmacist who stays late to make sure an elderly patient doesn’t miss a dose.

They don’t see the staff who hand-deliver meds during heat waves, storms, or pandemics.

They don’t see the quiet counseling at the counter when someone is scared, sick, or running out of hope.

And major news media rarely shows it because it doesn’t fit into a headline about profits, mergers, or quarterly earnings.

But this is healthcare.

Independent pharmacies are not relics of the past. They are frontline healthcare access points. They are safety nets. They are trust, built one patient at a time.

When systems fail, these pharmacies don’t disappear.
They unlock the door.
They answer the phone.
They show up.

If you want to understand what healthcare should look like in America, don’t start with an insurance executive or a corporate press release or your PBM steering you to the mail order pharmacy they own.

Start by visiting an independent pharmacy and a human being who knows your name.

What they actually do.

Let me help.

Your pharmacist catches the drug interaction that would've killed you.
Answers the phone at 9pm when you're panicking about your kid's fever. Counsels you on side effects your medications.
Knows your name, your meds, your allergies, your history.

Your PBM decides what drugs you're allowed to have.
Takes a cut of every transaction.
Pays the pharmacist less than the drug costs.
Owns the pharmacy competing against your pharmacy.
Has never once looked you in the eye.
Couldn't spell your medication without a database.

One is legally required to keep you alive.

The other is legally required to keep Wall Street happy.

Guess which one is going bankrupt and which one is Fortune 10.

CVS Caremark. Express Scripts. OptumRx.
Three companies.
80% of the market.
Zero pills touched.

They don't manufacture drugs.
Don't dispense drugs.
Don't counsel patients.
Don't hold licenses.
Don't assume liability.

They wedged themselves between you and your medicine and somehow became the ones running the show.

That's not healthcare.

That's a parasite in a quarter-zip vest cashing checks while your pharmacist skips lunch.

The middleman makes more than the people doing the actual work.

Here's the good news: PBM reform is one of the only issues left in America where Republicans and Democrats actually agree.

Left hates corporate greed.
Right hates market manipulation.

Thank you to all of you who helped out by contacting your legislators. SB444 passed the Senate without any objection and the House Insurance Committee and is headed to the floor for a final vote. Please contact your legislators one more time and ask them to support SB444 and share this post. Thanks again for your support of Independent pharmacies across our state.

First of all, I want to thank all of you who have chosen Thrift-Town Pharmacy as your pharmacy and entrusted us with your prescription and healthcare needs. I am posting this to ask for your help and support with an important piece of legislation that is critical to our survival as an independent pharmacy. Even if you’re not a customer of ours, but use an other independent pharmacy, I ask for your help too. This legislation, which is Senate Bill 444 (SB444), is currently being discussed and voted on in our State Capitol. You may or may not have heard of a PBM (Pharmacy Benefit Manager). PBMs are the “middle man” between your insurance company and your pharmacy. They set what amount the pharmacy is paid, what your copay will be, and what they will charge your insurance company. PBMs started as claims processors but have grown into Fortune 10 companies by exploiting the healthcare system, particularly when it comes to the cost of prescriptions, by charging your insurance company many times more than what they are reimbursing the pharmacy. SB444 will require the PBM’s to pay the pharmacy their cost to acquire the medication. Hard to believe we need legislation to require us to be paid the cost of a drug, but it’s true. If you use an independent pharmacy and you haven’t been told they can’t fill your prescription because of being reimbursed less than the cost of the medication, you probably will soon. I’m asking that you take just 1 to 2 minutes of your time and call or email your State Senator. Beth Mizell is my State Senator and Robby Carter is my State Representative. I have listed their contact info below. Depending on where you live, your Senator and Representative may be someone other than Sen Mizell and Rep Carter. If so, please contact them and ask for their support of SB444. Thank you in advance for your help in letting our legislators know the importance of the passage of this bill.
