Charles Kelman was born in Brooklyn in 1930. Everyone called him Charlie.
His father was an inventor. Never got the money. Never got the credit. Charlie watched that happen up close.
Music came first. Harmonica at 3. Then saxophone. Then clarinet. He wanted to be Benny Goodman.
He became an eye surgeon instead. Manhattan Eye, Ear and Throat Hospital.
Cataract surgery in those days meant a 12 millimeter cut. Almost half the eye opened up.
Ten days flat on your back in a hospital bed. Head packed still. Months to recover.
Charlie thought it could be done through a tiny hole instead. No hospital stay. Home the same day.
The John A. Hartford Foundation gave him a grant to find out.
He tried folding lens bags. Rotating devices. High speed cutting needles.
Every single one wrecked the cornea. He burned through most of his funding on failures.
Then there was the dentist. Larry Kuhn. The Kelman family dentist.
The way Charlie told it later, Kuhn lifted a long silver instrument out of its cradle and switched it on.
A fine mist came off the tip. The tip did not look like it was moving at all.
Kuhn put it on Charlie's teeth. Charlie felt a strange fine vibration. Heard a high pitched whine.
What is that thing, he asked. An ultrasonic probe, the dentist said. It takes tartar off a tooth without disturbing the tooth.
I knew this was the moment, Charlie wrote.
He went back for a cataract he had removed and they put the dental tool on it right there in the office.
Historians who went through the archives say it was messier than that. He may have seen the machine years earlier. The first tests did not work.
But February 1965 is documented. That is when he went to the company that built it. Cavitron.
There he found an engineer named Anton Banko.
Banko is the reason it worked. He put longitudinal motion into it so it would not tear the iris. He switched the tip to titanium so it would not flake. He built the irrigation and suction into the same handpiece.
Together they turned a tooth cleaner into an eye surgery machine.
March 23, 1966. First success. A cat's eye.
1967 Charlie was ready for a person. He found a patient whose eye was blind and painful and had to come out anyway.
The man agreed knowing it would not help him. It was for research.
Charlie screened off the operating room from anyone who might look in.
During that operation the cornea collapsed 30 to 40 times. The suction kept surging and pulling it down.
He got the cataract out. He wrecked the cornea and the iris doing it.
July 1967 he published the technique anyway. He called it phaco emulsification.
July 25, 1967 he and Banko filed the patent in a Brooklyn patent office.
Then his own profession came for him.
Surgeons called it ridiculous. Surgeons called it malpractice.
The National Eye Institute labelled it experimental. That meant insurance would not cover it.
1973. The Welsh Cataract Symposium. Surgeons stood up and presented bad outcomes.
They brought the damaged patients onto the stage as evidence.
Charlie had no academic post. No university behind him. A practicing surgeon with a saxophone habit.
The academic community treated him with open hostility for years.
And what bothered them most was not the machine. It was that he sent people home the same day.
He started teaching it in 1970 anyway. Between 1967 and 1973 about 3,500 of these operations were done. Charlie did 500 himself.
No institution would back him. So he went around them and spoke to the public directly.
1985 he wrote the whole fight down. Through My Eyes. The subtitle called him a surgeon who dared to take on the medical world.
Then the numbers turned.
1985 it was 15 percent of American cataract surgery. 1990 it was 50 percent. 1996 it was 97 percent.
Twenty five years from malpractice to standard of care.
George H W Bush gave him the National Medal of Technology. He won the Lasker Award in 2004. The Inventors Hall of Fame.
They named him Ophthalmologist of the Century.
June 1, 2004. Charlie died at 74. Lung cancer. He had fought it six years.
They held the memorial in a Broadway theater.
Think about where the answer was.
Not in his lab. In a dentist's chair. On a machine that had been sitting on the market since the 1950s.
Nobody else in eye surgery had ever sat in that chair and asked what is that thing.
His machine still runs. It runs in every eye theater on earth. Surgeons use the same technology on tumors of the spine.
Cataract surgery still takes about five minutes. You still drive home that afternoon.
Somebody you love is going to get cataracts. Maybe you.
They will walk in, get five minutes of a vibrating titanium needle, and go home that day seeing again.
Nobody in that room will say Charlie Kelman's name.
Almost nobody has ever heard Anton Banko's.
