Partnering with Appleton Area Schools and the City of Appleton is part of our community and healthcare mission! We were able to bring a Back To School Vaccination Clinic to Wilson Middle School, and Kaleidoscope Academy.

This month’s spotlight features Mosaic Family Health , serving families in the Appleton community. Mosaic Family Health has been a Reach Out and Read site since 2014, conducting over 500 Reach Out and Read well-child visits each year. Reach Out and Read is just one of the amazing supports Mosaic Family Health offers its patients. Located inside Fox Commons, the clinic’s central downtown location gives patients easy access via regional highways and the public transit system. The clinic is across the street from the Appleton Public Library and right down the block from The Building for Kids Children’s Museum, seamlessly integrating into the community.

Besides the clinic’s physical location in the community, another special feature of Mosaic Family Health is its community care coordinator, Jackie Gonzalez. Jackie works side by side with the team of doctors, nurses, and staff to help patients access and connect with community resources that will enhance their lives. Typically, she works with families after they have seen the doctor. “Kids definitely show me the book they get through Reach Out and Read very proudly!” she says. She has many connections to other literacy organizations in the community that she can refer families to, such as the Fox Valley Literacy Council, local libraries, and the Even Start Family Literacy program, run jointly by the Appleton Area School District and Fox Valley Technical College.

Reach Out and Read Site Coordinator and Clinical Supervisor Jenn Wentzel says, “I am proud to work somewhere that values a job such as hers, and her passion is evident.” With Reach Out and Read helping create trusting relationships between families and providers, and the community care coordinator supporting families in getting what they need, Mosaic Family Health is truly working to create a world where every child has the relationships and resources to learn and thrive.
