Before any lower eyelid surgery, I always take a close look at the surface of the eye. There are small changes that are easy to miss without magnification, but they can have a big impact on comfort and healing afterward. Sometimes addressing these issues first helps reduce irritation, improve symptoms like tearing, and set the stage for a smoother recovery.

A quick look at the removal of a symptomatic cyst on this gentleman's left upper eyelid.

The skin in this area is some of the thinnest on the body, so careful, precise technique is important to protect the underlying structures and achieve a natural-looking result.

For closure, I use plain gut sutures. These are absorbable, meaning they dissolve on their own within about a week. They are also very gentle on the eye, which helps minimize irritation and avoids the need for a suture removal appointment. Small details like this can make a huge difference in recovery.

I like to try treatments myself before I recommend them to my patients, so of course, I had to give REVVE a go.

A lot of β€œdry eye” isn't actually dryness or lack of tears. It's clogged oil glands and buildup along the eyelids that cause your tears to evaporate too fast. The REVVE protocol is designed to treat that at the source by clearing out bacteria, their biofilm and toxins, and helping the glands function properly again. This approach was developed by Dr. Rynerson, who personally trained our team earlier this year!

It was very tolerable, and within about a week, most patients start to notice their eyes feel more comfortable and less irritated.

If you feel like you are constantly reaching for eye drops (like me!), or suffering with gritty, sandy, burning eyes, it may be time to treat the source instead of just the symptoms.

May is Skin Cancer Awareness Month, and cancers around the eye are more common than many people realize. This 61-year-old patient had a squamous cell carcinoma involving the upper and lower eyelids. After Mohs surgery removed the cancer, I reconstructed the area the following day.

When tumors are removed, it can leave a significant defect. My goal is to carefully rebuild the eyelid so it can blink, protect the eye, and look natural again. This often involves using nearby tissue and skin grafts to restore both function and structure.

Early detection matters. If you notice anything new or changing around your eyes, it is always worth having it evaluated.

This patient came in noticing that one eye looked heavier than the other, with more hooding on the right side. Over time, that asymmetry can make the eyes appear tired.

With an upper blepharoplasty, I focused on restoring balance between the two sides while keeping his natural features intact. Even small differences between the eyelids can have a big impact on how rested someone looks.

These after photos are just 7 weeks after surgery, and you can already see a brighter, more open appearance. He looks more energized, but still completely like himself!

During her consultation, this patient mentioned to me that she felt vibrant and full of energy, but her eyes were telling a different story. Over time, her brows had slowly come down, creating heaviness over her eyes and deepening the lines between them. The puffiness in her lower lids added to that tired look.

To help, I lifted her brows back to a more natural position with a pretrichial brow lift and smoothed the under-eye area with a lower blepharoplasty. Now she looks more open, refreshed, and in sync with how she actually feels.
