She booked under-eye filler but her anatomy called for midface support first.
The fullness beneath her eyes is caused partly by fat-pad herniation, not simply hollowing. Placing filler directly beneath a prominent fat pad can add heaviness without adequately correcting the concern.
Instead, we treated the cheeks to restore midface support and soften the transition between the lower eyelid and cheek. Under-eye filler may be considered later, but only after reassessing the area once the cheek filler settles.
