East Lake Healthy Connections is a program of the East Lake Family YMCA. The program was created buy the East Lake Foundation and sponsored by a grant through the Jesse Parker Williams Foundation. East Lake Healthy Connections was developed to support the East Lake Foundation’s goal of community wellness for the residents of the Villages of East Lake, the students at Charles Drew Charter School, E

ast Lake Early Learning Academy and Sheltering Arms Early Learning and Family Center. Healthy Connections has a team of Family Health Advocates whose mission is to promote increased education and access to primary and preventive health care services to the residents and students in the East Lake community. The Family Health Advocates seek to enroll all eligible adults and children in an insurance plan, help each individual identify and establish a medical home, teach proper ways to use a medical home to increase overall health and provide education about chronic conditions such as asthma, diabetes and hypertension.
