Above and Beyond: A Name that Opens Doors to Improved Care

Despite being the established power of attorney, David was having a hard time getting updates on his aunt's care from the nursing home. He asked Michael to serve as an advocate. Having had months of frustrating experiences David did not have high hopes of improvement. When he introduced Michael to the care planner though, everything changed. Recognizing the name tag, the care planner exclaimed, "I have known Metta for years!" In short order, Michael was able to open the lines of communication. David was shocked at how quickly things turned around. "Wow! You all really do know everyone, don't you?"

Above and Beyond: Assuring Safe Hospital Discharges

After a difficult stay in the hospital Mrs. Green was ready to return home, but RN Care Manager Kathy saw the writing on the wall when the hospital waited until Friday afternoon July 3rd, a holiday weekend, to say she was ready for discharge. Kathy diligently reviewed the orders and took special note of the need for continuous oxygen. The medical equipment company was slow to respond and seemed unprepared, but the hospital discharge planner moved forward scheduling transport for Mrs. Green to return to her assisted living anyway. When Mrs. Green showed up and the medical equipment company hadn't delivered the oxygen as promised, Kathy was ready with oxygen concentrator she had gotten on her own, especially for Mrs. Green. This helped Mrs. Green comfortably and safety return home. Once Mrs. Green was settled and finally received her oxygen concentrator from the medical equipment company, Kathy was able to provide feedback to to providers involved.

Sadly, as Care Managers we know this to be true all too well. Clients moving from other states, or family members managing their loved ones care from other states are often surprised at how hard it is to maintain suitable healthcare in Georgia.

Georgia ranks 49th overall in WalletHub's 2026 study of state healthcare systems, placing near the bottom nationwide. The report evaluated healthcare cost, access and outcomes using 44 metrics. Georgia ranked 37th for costs, 49th for access and 43rd for outcomes.

The state also ranked low in key measures, including physicians, dentists and hospital beds per capita, as well as the percentage of insured adults and children. WalletHub said the strongest healthcare systems balance affordability, quality and access. New Hampshire ranked first overall, while Mississippi finished last.

Above and Beyond: Early Detection of Conditions

Emily had worked with Mrs. Smith for several months, helping her manager her medications, doctors appointments and remaining engaged in her community. Emily was one of the first people to notice Mrs. Smith's cognitive decline. By engaging medical providers, they ruled out acute conditions. Mrs. Smith eventually was diagnosed with early stage Alzheimer's. Emily knew that early detection of Alzheimer's is crucial in qualifying for the newer and more effective infusion treatments. Mrs. Smith was able to being treatments and, with support, her functioning and quality of life have improved.
